I had the same fears, the same bottle addiction. We switched to sippy cups cold turkey and put away all of the bottles at one time. It took about a week of an unhappy DD but it worked.
Once the bottle itself was gone we were able to get rid of the morning milk. Still working on the before bedtime milk now so it is a slow process for us.

Hmmm that's a good point that it may just be a habit at this point. He eats a good amount during the day. He's on the lower end for weight (25%), but he has 3 meals and 2 snacks and he usually snarfs down his dinner so he's probably not eating because he's hungry.
The logistical issue in the morning is hard because he wakes up at 5:40 every morning and we usually let him chill in his crib til 6:15 when we are ready for work, and I like the idea of distracting him by taking him straight down to breakfast, but if he eats breakfast that early he's going to want lunch at like 10:30. Usually we give him the bottle right off the bat and then he can hold off on breakfast until 7:30 or so, which is much more manageable as a breakfast time. I suppose we could try the cup and if he's not having it we could put him in his highchair and give him some cheerios and milk in a cup as like a pre-breakfast stnack.
Rachel, what suggestions do you have for changing the bedtime routine? We usually do dinner, then bath (which is about 20 minutes since he loves playing in there), then we brush his hair and I sit in his rocker with him and give him his bottle, brush his teeth and go to bed. What would you change up to eliminate the whole, "Time for bottle now" thought after bath?

I have zero advice.. just hugs!! My DS is very dependent on his bottles, too.. I have no idea how we're going to cut back!! You actually just inspired me to post about my worries as well!! PS your morning and bedtime routines are identical to ours.. I have no idea how we're going to switch it up to break the "habit"!!
DS is a poor eater so we need to get as much into him as we can calorie and protein-wise - so he still gets "milk" (formula) after he is up and dressed and about 30 mins before bed. We had the same 2 bottles as "stragglers" and I was concerned, but one day in the morning I just handed him a sippy instead of a bottle, he looked at it, looked at me, and just drank it. We never went back to bottles in the morning. He eats breakfast around 45 minutes later, with the nanny during the week and we time breakfast similarly on the weekends.
When the morning worked out so well, we started referring solely to "cups" and not "bottles" and within days he started requesting "milk in a cup" - so we chucked the remaining evening bottle and started giving him a sippy in the evenings too. It was weird - for a kid who is as regimented and picky as ours, he has never once asked for a bottle since the transition.
He takes between 4 and 8 ounces (depending on the size of his dinner which can vary wildly) before we start our bedtime routine. We brush teeth as part of that routine to hopefully avoid the dental nastiness.
