supplementing with formula

thanks to advice from you guys, i started supplementing this weekend with gerber good start formula because i can't seem to pump as much as my baby needs lately. 

 as i am packing his bottles for the day now, how much extra do i need to include? he usually takes about 10 oz breast milk and lately ive been pumping about 5-6 oz.  so do i need to add 5 oz of formula to make it to the 10 oz, or do i need to add more than that?

    I do the same amount of formula as bm if I'm supplementing. So if she usually gets a 5oz bm bottle.. I'd do a 5 oz formula bottle or mix the two... 2oz formula 3 oz bm.

    Same as PP.  We were doing 12oz BM, so now it is 8oz BM, 4oz formula.  Although I think we are going to be 100% formula in the next couple weeks and that will mean more oz...not sure how many though.

    We started and just do the same amount as she's getting roughly 20 ounces total for the day and anywhere from 4-8 ounces of that is formula.  I did however raise her bottles recently - but that was in hopes of getting her to sleep more during the night(and its working :) )  I too am not sure if we move to 100% formula during the day how big to go - but I would say start small and increase as needed?
