High-Risk Pregnancy

New to GD - chances of induction?

I haven't been officially diagnosed with GD yet (going in for my 3-hr on Wednesday) but because I failed the 1-hr miserably (196), I know the chances are high that I'll end up with GD.  I'm already trying to prepare myself mentally - researching, looking up low-card recipes, etc. - and am wondering about the chances of my OB inducing me.

Obviously this differs for everyone but am curious as to all those with GD, has your OB indicated there is a strong possibility you will be induced by or before your due date?  I'd love to avoid an induction but will obviously go with what my doctor thinks is safest.

I'm also wondering if this might have any impact on trying for a med-free birth... as in, does having GD, require more monitoring or medication during labor?

TIA!

Re: New to GD - chances of induction?

  • I had GD with my first and now again with my second pregnancy.  I was induced the first time 3 days before my EDD because they thought my DD was getting big based on growth ultrasounds.  The ultrasound was not accurate at all, she ended up only being 6lbs 13oz.  They did give me pitocin get things started, but then my body kicked in to gear and the pitocin was turned off and I was med free from there.  Really there was nothing any different than a non GD birth, though they did check my DD's blood sugar about 6 times in the first day or two.  Good Luck!

  • With my first pregnancy, my OB said she wouldn't let me go past my due date. I ended up induced at 37 weeks due to pre-ecclampsia though (not GD). The induction failed and I had a c-section. This time my repeat c-section is scheduled at 39 weeks but that's not due to the GD, it's just standard for repeat c-sections. My baby and I had no problems related to GD after birth. My OB did say that GD puts women at a higher risk for developing pre-ecclampsia, so that's something to be mindful of.
  • It depends on a lot of things. How well your GD is controlled. How your GD is controlled diet, oral meds, or insulin. If you measure large. Your OBs guidelines that they go by.

So far I am diet and exercised controlled. As long as I stay this way then I won't have any NSTs or ultrasounds and I will be able to go to 41 weeks. My OB won't let anyone go past 41 weeks. This being said, my OB seems a lot more relaxed about GD than others.

  • I'm on insulin and probably being induced this week, right after I hit 39 weeks because my blood sugar has not been great and they are worried about my placenta quality. They will closely monitor you near the end and let you know.

  • Both with my son and this pregnancy (both GD) I was told I would not be allowed to go past 39 weeks. I was induced at 38w 2d with my son due to him failing the NST. Other than the Cervadil, I had a med free delivery.

  • I had GD with my first, and was not induced. I gave birth spontaneously and vaginally at 41 weeks and 1 day. my son weighed 7 pounds and 11 ounces. I only had an NST after I went past 40 weeks. I was with midwives and was dietcontrolled.

With this baby, I am taking 5 mg. of Glyburide at night, but I'm still planning natural birth. There is no talk of induction barring an urgent medical reason. Baby is measuring 1 week ahead, or about the 60th percentile, is head down, and anterior at 32w4d, so no one is concerned and we are on track for another vaginal delivery sans induction.
  • Right now the plan is that I will be induced between 39 and 40 weeks.

If you are diet controlled, you should be allowed to go to your due date assuming no complications.  My midwife would let me go to 41 weeks assuming good sugars that I recorded and a good a1c (which was 4.5 at 39 weeks).  I had an NST and BPP scheduled for 40 weeks, 3 days, but went into labor at 40 weeks, 2 days.  I didn't get to the hospital until I was 9 cm and had a med free birth.

It is possible, just keep as tight of control on your diet as you can and labor at home as much as possible ( but not too long)!

  • It's going to vary a lot based on your drs and situation.

With my first, my drs said absolutely not to letting me got past 39 weeks. That was their standard for all GD patients, diet or med controlled. I was on insulin and induced the evening of 38w6d. Delivered vaginally at 39w2d, after a really long induction. I did have an epidural due to length of labor and horrible, horrible back labor. I also had to be on an alternating glucose/insulin drip during labor, but I had crap care in that pregnancy and my blood sugar was not where it should have been.

With my second, drs were open to letting me go into labor on my own provided I wasn't on meds and went before 40 weeks. I was on insulin again by 18 weeks, and induced the evening of 38w6d, delivered vaginally at 39w. Another epidural for back labor, but no glucose/insulin drip needed.

I feel pretty confident if my labors were shorter or my kids weren't "sunny side up" I could have gone without the epidural. I have heard of many women who are induced and go without any meds besides the cervidil/pitocin.

  • It seems that every OB/midwife handles the induction date a little differently. I'm on my 3rd go-round with GD, and stayed pretty well controlled with diet and then a small dose of oral meds in the last few weeks. My midwife does not want me going much past my due date but will not induce before 40 wks if all looks well (well controlled sugars, baby measurements, NST results, fluid levels, etc).  With my 1st, I went into labor on my own 2 weeks early so it wasn't an issue. But with #2, I was induced on my due date, and had a successful pain-med free birth (though the Pitocin sucked). With both labors, I didn't require any extra monitoring during labor, they never even bothered to check my sugars. But again, I was mostly diet controlled. If I was on insulin, it might have been a different story. After the birth though, they had to closely monitor my baby's sugars for 24 hours, the first 12 of which they insist they have to keep them in the special care nursery. I was NOT happy about that, and both kids ended up completely fine.

  • I was hoping to have a natural birth with my son... I had GD with him as well, I was induced at 39 weeks due to placenta breakdown and my ultrasound indicated my son was pretty large especially due to my small frame.

I ended up having a c-section as I was on pitocin for 24 hours with no progress. My son weighed 8.5 which is what the ultrasound indicated.

    I think everyone is different...
