We had just got home from the grocery store and LO was getting his diaper changed. He doesn't full roll yet, so he still lays on the middle of the couch when we are within arms reach. FI turned his back for just a second to grab a diaper, and LO had rolled off of the couch, onto the hardwood floor, which just so happened to have one of DS's plastic cell phones on it. LO hit the corner, and his head is cut from it. It's hard to described other than it looks like the corner of the plastic cell phone had punctured his head. It looks kind of like a blood blister, but it's not openly bleeding. The area surrounding it is red, probably about the size of a silver dollar.
I called the nurse line and am waiting to hear back on what I should do, though I'm sure they'll say, "Bring him in to get checked out." (Which is what they say for literally everything)

Falling alone, I wouldn't worry. A silver dollar sized blood blister would have me in the ER though. I wouldn't wait, just go in and get it checked. ETA: Is the big area the blood blister or just redder than normal skin? Just redder, that wouldn't concern me so much...
The punctured/blood blister part is little. The silver dollar description is the red area around it. I'm just more concerned about the cut. Their heads are so soft.... My mind just "goes there" when something happens. Like, "Oh my gosh. He's going to die."

I agree. And don't stress about the previous poster saying "brain damage". It is far too early to start ringing the panic button.
I would take LO in for a check. In my experience if LO is not exhibiting any worrisome symptoms and is otherwise healthy they won't initially do a CT scan. They will likely give you explicit instructions regarding watching a head injury, and you should follow those, bringing LO back to the urgent care or ER if you are concerned.
The issue isn't really the break in the skin, but a trauma hard enough to break the skin could cause damage/injury to the skull/brain and should be evaluated.
Btw, my first son(13 years old) rolled off the couch at 3 weeks, I took him in to get checked.
