IUI Opinion

We are moving to IUI this month. Today is CD1.  I have very regular cycles and O day 14-16, with a pretty consistent 14 LP. 

 Would you recommend doing a unmedicated IUI or medicated with clomid, monitored, 25 mg for 5 days + trigger?  Or something else?  These seemed to be the 2 options that I was given, although I'm willing to advocate for another option as well.   

 Or am I not going the right direction with IUI as our next step?

  • I attempted to do a couple natural (unmedicated) cycles, because I too O regularlly. We finally agreed to Clomid to increase our chances. I'd say its up to you, as well as your DH.

    I'd also consider cost. Do you have coverage or are you OOP?

    Personally, we went right to the meds with our first IUI (our current cycle). We are fortunate enough to have excellent insurance which covers a lot of the cost, so we figured why not. Anything to improve our chances! Good luck in coming to a decision that you feel comfortable with!

    Dx: Poor Embryo Quality, Arcuate Uterus, Poor Uterine Blood Flow, Mild Endo, 
           Protein S Deficiency, Sjorgen's Syndrome 

    IUI #1-5: BFN
    Laparoscopy & Hysteroscopy: minimal endo, partial septoplasty
    IVF #1: 10R/6M/6F ~ Day 3 ET = BFN
    IVF #2: 14R/9M/5F ~ transfer canceled ~ all embryos arrested at 1-2 cell stage
    IVF #3: 9R/5M/5F ~ 1 frosty!
    IVF #4 (FET #1): BFN

    IVF #5 (DE IVF #1 with Dr. KK protocol): Currently PREGNANT!!!!!!
    Synthroid + Prednisone + Metformin + Baby Aspirin + Supplements Galore = 15+ pills a day
    Lupron + Lovenox + Delestrogen + IVIG + B/W = 2-5 pokes a day
    19R, 17M, 17F - transferred two Grade A blasts 11/16, four frosties!!!
    Beta #1 11/24 (13dpo/8dp5dt) = 367 ~ Beta #2 11/26 (15dpo/10dp5dt) = 709
    Beta #3 11/29 (18dpo/13dp5dt) = 1,997 ~ Beta #4 12/1 (20dpo/15dp5dt) = 3,403

  • It's going to depend on how aggressive you can and want to be. It doesn't seem like they would be a waste of time in your case.

    In my case, we chose to be as aggressive as we could and I liked having my cycle controlled by meds and triggers for timing things out.
    Me: 37, DH: 35 :: TCC since 2/11 SA: Perfect! CD3  HSG = Blocked Right Tube
    April- Femara 2.5mg + Trigger + IUI = BFN   May- Femara 5mg = CX - No Response on Left = BFN
    June- Femara 7.5mg + Trigger + IUI = BFN  August- Lap & Hysteroscopy = Blocked & Partially Blocked Tubes
    September- Femara 5mg = CX - No Response on Left = BFN  October- 100mg Clomid + Trigger + TI = BFN
    IVF # 1: Stims 11/30 ER 12/12/12! (10R, 10M, 8F, 2T, 6 F) :: Beta #1- 176  c/p @ 4w4d
    FET #1 February 26th :: Lost 4 to Thaw, Transferred 2 = BFFN
    IVF # 2 Stims 5/10 ER 5/21 (15R, 13M, 13F, 2T, 7F- 6d3 & 1d5) :: Beta # 1- 15 c/p @ 4w
    FET #2 Cancelled, Right Tube Developed a Hydro  8/28 Hydro & Scar Tissue Removed  Cleared for FET
    FET #2.2 Scheduled for September 20th
    2 Thawed, 2 Transferred! Beta #1- 96, Beta #2 906! :: EDD June 10th
    2015- 2 failed FET. We are done

    I have coverage up to a $15,000 lifetime maximum, so I figured might as well as  be aggressive.

  • I asked my RE about unmedicated IUIs when we were going over all the different options and success rates, and he said they generally don't do them because they don't find the success rates to be any more than a TI cycle.
    Me: 31, DH: 31, TTC since April 2011. "unexplained" dx
    Initial testing through OB in August 2012, including HSG and SA
    Sept 2012 BFP, m/c at 7weeks.
    Met with RE March 2013
  • We moved straight into medicated IUI when we started treatment.  I did have one unmedicated IUI, thrown in last minute during a testing cycle.  I am currently doing 50mg Clomid + trigger + IUI.

    September 2012: Start IF testing

    DH (32): SA is ok, slightly low morph, normal SCSA  Me (32): Slightly low progesterone, hostile CM, carrier for CF, Moderately high NKC, High TNFa, heterozyogous mutated Factor XIII, and +APA

    October 2012-May 2014: 4 failed IUIs, 3 failed IVFs, and 1 failed FETw/donor embryos

    November 2014: IVF w/ICSI #4 Agonist/Antagonist with EPP and Prednisone, Baby Aspirin, Lovenox, and IVIG for immune issues.  Converted to freeze all due to lining issues.  2 blasts frozen on day 6!

    January 2015: FET #2 Cancelled due to lining issues

    April 2015: FET #2.1
