Sh*tty monitoring appt : ((

Just got back from monitoring (day 6 of Gonal F). I only have 4 follicles that she thinks can make it.  Angry

The nurse started talking about being converted to IUI etc. WTF. I am so upset right now.

Does anyone have any idea if it is worth me going on a higher dose now or is it too late? I am on a low dose (187.5).

I had:

L - 1.7, 1.4, 0.8

R - 1.5, 1.3

  • I had my appointment today too. My nurse said a lot of times they don't even measure on day six...? She did with me but I think there could still be hope of them just upping your meds...maybe someone who has already gone through the process before will have better insight. But I'm rooting for you!

  • I'm not sure if it's too late or not, but depending on your dx, maybe ask about adding Human Growth Hormone to the mix. I'm DOR and it's something RE told me about before we got started on this cycle (which just got converted to IUI yesterday because I produced a few mature follicles too fast...ugh!). He wasn't convinced I'd need it, but said he'd assess after 5 days on stims. It's supposed to kick start growth (I think) and contribute to egg quality. Seems to me there might still be time to add it, or to up your meds. I'm no expert, though.

    Began TTC #1 in Sept. 2010. I was 33, DH was 36.. Dx with DOR and slight MFI (8/4/11).

    Test results (after first RE visit 6/21/11): 7DPO b/w: Progesterone = 11.3; CD3 b/w: FSH = 10.1 mUnits/mL, E2= 52.0 pg/mL, AFC: 6; SIS Ultrasound: Uterus great, tubes clear! AMH: 0.3 

    IUI#1 June 2012 (Follistim/HCG trigger): BFN 
    IUI#2 July 2012 (Follistim/HCG trigger): BFN 
    IUI#3 August 2012 (Follistim/HCG trigger): CP (Beta #1: 4.61, Beta #2: 1.0) 
    IUI#4 October 2012 (Follistim, ovulated before trigger, missed IUI, converted to TI): BFN 
    Jan. 2013 New RE
    IVF #1 June 2013 MDL Protocol. Converted to IUI#4.1. (High E2 and 3+ mature follicles at first monitoring.) BFFN
    IVF #1.2 August 2013 Stop Lupron Protocol with Human Growth Hormone added. 17R, 14M, 9F (with ICSI), 2 embryos (decent quality - grade 2) transferred on day 3, 2 blasts made it to freeze. Beta 8/26. BFFN :'(

    FET #1 December 2013 Last try! Transferred 2 blasts -- graded 3AB and 4BB. Beta #1 (12/27/13) 530. Beta #2 (12/30/13) 1876. BFP! One bean. EDD 9/3/2014!

    We welcomed the most beautiful baby girl into the world on September 11, 2014!

  • A lot will depend on your e2 as well.  If you are getting too high they won't want to increase your dose, but adding the low dose HCG could be an option.  I don't think you should consider yourself out of the game yet.  While it's not going as you hoped, your doctor might still be able to get a few more to catch up.  Sorry it's not what you wanted to see today.  ((hugs))

  • Just curious if those sizes are in cm or mm? In assuming cm because if so those follies seem pretty decent size at this stage...do you have other smaller follies that could catch up? That is a pretty low dose you are on. Its tough to say whether you should increase or not because if you have 4 dominant follies it might be tougher for those little ones to catch up. Im so sorry your appointment didnt fo as planned.

    It's in cm. And yes, I think it is too late to up the dose and get more for the reasons you said. They did not mention any other ones except the ones I put on my original post.

    Here is the kicker - in April (after my cancelled IVF.1) I went for a second opinion. The RE there was really shocked that I would be on such a low dose of Gonal and said she would have put me on a much higher dose to "wake up" the ovaries. I asked my RE about it and she said no, that dose is fine for you. And now.. my IVF is going to get cancelled and It could have been prevented. I am so mad and upset.

  • I don't have any advice on the size but with my fresh we thought we were getting four the whole time until ER and we got 6.

  • I would think that they would continue you on the same or higher dose as it'll take a few days for the lead to mature anyway before you trigger.  What protocol are you on?  Are you taking ganirelix?

    Me (37) DH (39); PCOS changed to Unexplained, changed to DOR in 2012 (finally a correct diagnosis!); 
    Started TTC 2009 with RE after 6 months.  
    Clomid + Trigger x2; 
    IUI + Femara x1,
    IUI + Follistim x2;
    IVF #1 (MDL) February 2013- BFN.
    IVF #2 (antagonist) May 2013, First BFP of my life. 
    Identical twin miracle BOYS (!!) headed our way- due date is technically 2/4/14 but c section is scheduled for 1/7!

    No ganirelix. Long lupron. I was on lupron 12 March - 5 April then cancelled cycle and Bcp from 5 April to 19 May. Started lupron again 15 May.

    They are increasing my Gonal F to 225. I'm worried it is too little, too late but not much I can do about it now.
