IUGR or SGA

Question for anyone else who is IUGR or SGA .

At my 30 week scan, baby measured totally fine and in the 60+% range for overall growth.  

At 34 wks, he had dropped to 40% overall (4 lbs 3 oz) and his abdomen only measured in the 8th, (head and femur remained in the 40s).  The MFM Dr mentioned that it looked at though we were looking at asymmetric IUGR.  He wanted to measure again in 3 weeks (this is the normal interval between scans).

Yesterday, at 37 wks, we are down to the 19th percentile for overall growth (5 lbs 3oz) and the abdomen is at 6%.  (He does have fat legs, btw, so that can't be too bad).  The sono tech said that the diagnosis is SGA, which I find to be strange, given that he is measuring over 5 lbs.  I didn't think to ask to Dr about this diagnosis change, he was focusing more on the delivery/ internal exam findings.  My Dr is suggesting we wait another week, scan again (as well as the NST) and try our best to get to 39 wks in hopes I go into labor naturally.  (This is also on target with what I want to do since my son came 10 days early and I'd prefer that course over induction or CS.)  

I totally trust my Drs, they run the antenatal testing center for a high-rated hospital with a top-notch nicu.  I'm a little worried things are starting to crap out here at the end and I want to see if anyone else is getting a similar directive from their Dr.  I have upped my diet to 2000+ cals, (high protein) and rest 2 hrs every day, per my instructions.  I've also been told the importance of calling in with any discrepancy in my kick counts (3 x per day).

  • I don't have any experience with IUGR or SGA, but I wanted to share an interesting perspective on weight percentiles that my doctor has. With the twins we have gotten lots of growth scans, and they are growing well but the percentiles keep going down. I asked my doctor about it and she explained that the farther along you get the more "competitive" these weights get. This is because the sickest, smallest babies are born earlier so the farther along you get the babies that yours is being compared to are bigger and healthier. Does that make sense? Anyway, when she explained it like that it really eased my mind. Sending tons of thoughts and prayers that your LO continues to grow so that you can aim for 39 weeks! GL!

  • Thanks for the info.  I know he's measuring smaller than my son but I didn't think he qualified for SGA given his overall weight.
  • I don't have experience either, but only a dr can diagnose, not a tech. She is there to get the scans, not interpret them no matter her experience. I'd just ask the dr for clarification on the diagnosis.

  • I have to read more on SGA but my baby is IUGR and SGA and I read a ton on induction vs waiting it out for IUGR babies.. Different situation for us in that our baby is symmetrical and just now 5lbs at 38weeks and 1 day. She is in the 2nd percentile for weight so they are inducing me today or tomorrow at 38weeks and 3 or 4 days... I find the pps doc explanation of the growth curve very interesting though and wonder if everything else is normal with NST and AFI, BPP etc then why induce? My dr said that they believe my placenta isn't doing all it should but how can they say that when all the tests come back good? I'm just confused by it...

  • I hear ya. My AFI is good, cord flow is good and from lwhat little they can see, no calcification a on the placenta. Doesn't make any sense.
