DH and I had an appointment with our doctor this morning and he was surprised that she hasn't started to grab/reach out/hold things. Our LO is 10 weeks old tomorrow and I thought we were really on track with the milestones. When did your LO start to hold things? What did you do to promote grabbing/holding? Should we be worried?

This. DD is 11 weeks today. She'll hold on to things if I put them in her hand.
I think at this age (also 10w tomorrow) babies are really working more on their gross motor skills. Grabbing onto toys would be fine motor skills (from what I know of gross and fine). LO still makes fists 95% of the time. Some of the few times her hands are fully open is when she is sleeping.
When she is on her activity mat she bats at the different hanging toys. She has a plush doll that is usually with her in her carseat/bouncer and she'll move that around with her fists or arms, but she isn't really using her fingers, yet.
I wouldn't worry.
