The infant insert is meant to keep baby's head secure when they don't have the strength to hold their heads up yet. It's to keep baby from flopping around. Not sure about this flat spot theory.
ETA: I bought myself the maxi cosi mico as it is the only car seat that fits in my tiny car and also allows FI to actually fit in the front seat as well. I love the styling and colours, and the fact that it is so lightweight.
The head insert on the micro is nearly identical to the one on the pria. I don't think it it has anything to do with preventing flat spots. If anything it provides support for the head without adding material directly behind the head, which could push a tiny baby's head forward.
I used a mico with my DD for about 10 months and liked it a lot. It easily installs with and without the base and has a nice smooth handle. The handle can be in any locked position in the car as well.
Unless you're planning on putting your baby in the carseat for hours and hours a day, I don't think you really need to worry about it giving him or her a flat head. Go with a safe carseat that fits in your vehicle and don't worry about the flat spot thing.
