I don't think I've ever been in that situation. It would depend on the child.
For DS's birthday, there were a couple of people who would have come to the party that were out of town, and they gave a gift. We were pretty close, though.
Depends how on the ball I am. This has only happened on a couple of occasions, the first was SO's cousins kid, so I think I did pick something up because we were out of town. The second was a friend of DD's from playgroup. I had every intention of getting something and dropping it off, but it didn't happen.

LOL I didn't even realize I had posted this...I typed the question and then DD was trying to grab the laptop when I was putting it away, I guess it got posted in the scuffle!
Anyway, I generally do not, except for a select handful of people with whom I'm very close with them and their kids. These are mostly friends, less family members.
The only people I could ever see giving DD a gift after not attending her party are our parents, and maybe my siblings. Which is unlikely, because they'd be at the party, barring unforeseen circumstances.
