I had my long-awaited follow-up with the RE today! and moving forward with treatments; I finally finished all of the testing: full cd3 b/w and u/s, saline sonohystogram, hsg, 2 hr gtt, prolactin, dhea-s, and day 21 progesterone. DH had his SA, which was pretty good, just slightly low morphology. my dhea-s and prolactin levels were repeated for slight elevations.
This was an interesting consult because he put the options on the table and let me choose. If I hadn't been reading this tttc board, I would have been truly lost. for that, I owe all of you a million thanks!!
Diagnosis: PCOS and elevated dhea-s. (non-IR pcos) I was worried about IR because my father is one year post type II diagnosis - though he is overweight and not the healthiest eater. my cd21 progesterone was only 1.7 ![]()
His choices for me:
1. unmonitored clomid (why would he even offer this?!)
2. fully monitored clomid (called "aggressive clomid" by his office)
3. fully monitored clomid plus trigger shot
4. fully monitored clomid plus trigger shot plus IUI
he spoke about using letrozole or injects in the future should the above not work.
He will treat the elevated dhea-s with 1mg dexamethasone/day
I chose #3! If this doesn't work, I hope to do letrozole and then move to IUI.
thanks for reading this novel and please feel free to comment! thanks!

What she's saying is that P4 should be tested 7dpo NOT on CD 21. So unless you know you ovulated (by charting or bloodwork) on CD 14- your P4 number is worthless.
I'm skeptical of an RE who thinks monitoring is aggressive.
