Medical Release Form

DH and I will be leaving on MOnday for a cruise without LO. Does anyone have a medical release form or generic doument they can share with me or tell me where they got it. We want something saying that our parents are allowed to provide whatever medical care might be needed in case of an emergency and also if we are not able to care for our child or die (god for bid) they will be the caregivers. I know if we had a will, we would probably have something like this but we have not gotten around to that yet.

    Then, I typed up a page with:

    DD's name
    DOB
    Our names, address, phone numbers
    2 Emergency Contacts with addresses and phone numbers
    Physician's name, address, and phone number
    Insurance info: Insurance company, Policy holder, ID number, group number, company's address & phone number, etc.
    Allergies
    Medications & any other important medical info.

    You could also make a copy of your insurance card.  It's all probably overkill, but it's not a lot of work and nice to have on hand.

    For that I think you need a will. You can draft a pretty simple one for free online and then have it notarized.

  • You may also want to look into contacting your pediatrician. I know my peds office has us add people to my son's record that are authorized to bring him in for care. This way they already have our permission to bring him in to be treated and to use our insurance for him as they normally would.
