When I worked in Cust Serv, the mute button suddenly didn't work when Floor Support took an escalated call. She called the guy a ton of names....and ended up getting fired she had been there for 12 years.

Oh wow, that sucks.
I mean it was pretty bad what she said. She was so frustrated that she thought it was okay to say things like "I wish he would drive into a tree and die" "This guy is a stupid MoFo(used full word though), etc etc.
BTDT. I also complained about a department in IM to what I thought was my coworker in my department. yeah it wasn't. wrong window. I just played it off like I meant to complain to him and just asked, what's up are you guys super busy up there.
I haven't been caught, but I know people who have. In fact, it just happened to the guy who sits next to me today. I broke the habit of bishing about customers when the phone is muted a few years ago.
I did, however, have an embarassing moment once when a coworker warm-transferred a customer to me but didn't let me know she was on the line. She was BSC and totally heard me say "Oh no. Not her again." Whoops. Fortunately, my supervisor was cool about it.
