When my girls had their tubes put in (they were 14 months and 10 months) they could not have anything to eat or drink after midnight - not even clear liquids - because they were going under anesthesia. We just put them to bed as usual and let them sleep as long as we could before waking them to go to the hospital. We found there was enough new stuff and so much activity and so many people coming into tell them how cute they were :P that they were distracted enough that they didn't get too cranky about being hungry/thirsty. As soon as they were in recovery we could give them something to drink.
So yes, I'd just let him sleep. I find hungry kids are easier to distract than over tired kids, so sleep would win for us.

I'm trying to recall what we did for DS1 when he had to be put under for dental work a few yrs back. It was the same thing--- no food, etc. I was totally stressing about it but actually he did great--even with the surgery being delayed a couple of hours.
So not even water in the morning?? Are they putting him under?
Yes, you could try waking him up at 5:30ish and give some water--and maybe rock back to sleeep/ maybe you'll get lucky and he'll go back down for an hour or so. My other advice is to bring some "new" toys/books/etc with you so that he's not wanting food/drink--but is busy with other stuff.
