Do what for a fever?

I am giving B baby Tylenol every 4 hours like the doc told me but at about 3 hours the fever spikes again. He gets the chills and his lips get really red. Is there anything I can do?

Can you alternate with Motrin? I also would strip down to diaper and use a cool washcloth if allowed and if you need a blanket something like a Aden and Anais muslin one. I hope he feels better soon!
