the mystery of the endometrioma

Hey  all, I've been away for a few months trying not to think about all this because RE had me sidelined with birth control for several cycles before trying IVF this month. Endo and low ovarian reserve are my issues. Had my baseline yesterday and a persistent endometrioma continues to show on my affected ovary. Doc says it will stand in the way of most of the egg retrieval on that side. I?m so confused. U/S shows it but when my previous RE did surgery in Dec it wasn?t visible. I don't think I can chalk it up to forming after surgery because U/Ss before surgery showed it too. Can an endometrioma be inside an ovary?

 

35yo, DH 40, 5 yrs TTTC. Normal HSG and SA. Challenge is low ovarian reserve (AMH=.16) due to moderate endo. Two laps, 2 IUIs with injectables, no baby. Trying first IVF cycle 4/13. Actively seeking egg donor.

    I have never heard of an endometrioma forming inside of an ovary. Before my lap, I had 2 suspected endometriomas around each ovary, but were removed during my lap.

    My previous RE was planning to drain it but didn't do anything during lap aside from removing some scar tissue because there was nothing there. She said I had no active endometriosis, just the scarring. I have the pictures to back her story.
