  we change the nipples when we need to increase the size (usually every 3 months) and we haven't changed the pacifiers yet.  she just turned 6 months, I guess we should look into the 6 month ones or just cut them out completely.

  Same here, never replaced nipples other than changing sizes. With paci's I don't know why but my LO has a knack for losing his so we've probably bought him a good 10 of them and he is onto a new pair of them right now.

  I never have replaced them either....I have only gone up in sizes. All of them look fine to me though...

  I replaced the pacifiers at 6 months bc the 6 months ones are actually bigger which seemed like am important distinction to me for some reason. I still haven't replace any bottle nipples. As a breast feeder, I like to keep the bottle flow low so I really
