4 month old constipated

I'm a FTM and need some advice! My DS is 4 months old and is constipated. All day he's been trying to poop every hour or so and it's only a very small amount and it's a hard consistency. He is getting really red in the face and is crying when he's trying to push. We have tried to add extra water to his formula and I have been doing belly massages, moving his legs and we have tried giving him a warm bath. Any other suggestions?

We've been doing 2 ounces of pear or prune juice twice a day per our pedi. It's kept DS regular since we started with it.

We did the same thing with the apple/prune juice and it worked great!
