We had the district SLP come in 3 times to evaluate DS1 (he turns 3 in June). She mailed out her final findings and we just received them yesterday.
1) It says he has a diagnosis of Fragile X with cognitive disability. He doesn't. He has an x-linked intellectual disability, which is very different than Fragile X. DS1 doesn't have the dysmorphic features, has severe hypertonia and so far isn't showing signs of ASD, just moderate intellectual disability.
2) She wrote that when he's excited he goes into flexion. This is incorrect. He goes into extension. I feel like she wasn't qualified to make this observation if she can't report it correctly. Very few kids would go into flexion, most would go into extension so flexion isn't a common answer.
3) She has his birthday wrong through the entire thing.
Do I call and have her change these things or just let them be?

It'd be a big deal to me. Number one, I think it's disrespectful to have inaccurate information. Number 2, I think people tend to believe "professionals" more than parents. So you go somewhere and take that paperwork along, it makes it look like
