I am trying my best to come to terms with our loss but am really having difficulty figuring out where things went wrong.
Quick summary: My clinic has a 63% live birth rate for DE in 2011. Our donor stimmed for 8 days (a little short but RE doesn?t think that is the issue). Out of the 12 mature eggs retrieved 8 fertilized with ICSI, 4 made it to blast. The two that were transferred were ?excellent quality? (they do not use a number system) and I was given a 70% pregnancy probability with a 40% chance of twins. Our donor has had two prior successful pregnancies, two freeze all cycles, and one cycle that resulted in m/c which we were told was due to a sperm issue.
DH has had sperm issues in the past. When we started this journey many moons ago our dx was male factor as he had 6% morphology. After consulting urologists and faithfully taking supplements (which he is still on) his SA was brought into normal range. The last DNA frag test he had was last April but at that point his fragmentation was 22.2%. Our RE does not seem to think that he is the issue but of course admits that it is possible. He seems to feel that there is something going on with me. Right away I pointed to my fibroid and adenomyosis. Again, like I have been told by several other RE?s, he does not feel the location of the fibroid is a concern and he feels that adenomyosis only contributes to failures when it causes a thin lining ? mine was 13mm at transfer. He seems to think there is something going on autoimmune wise. This is where I am getting really perplexed. I have had NK cell testing, ANA testing, anti-cardiolipin testing, lupus anticoagulant testing etc. and everything came back in the normal range. Part of me wonders if something went wrong with the cycle that I am just not seeing and pointing to me is the easiest out. Am I being too cynical? There were two points in the cycle where my clinic made errors. The first one was they did not go over the instructions with our donor regarding when she was supposed to stop BCP which resulted in her staying on BCP five days longer than scheduled. The second mistake (a big one) was by the same nurse who ?forgot? to check off PGS (which we had paid for) on our lab slip. Maybe we would have some answers had that not happened.
I guess I just want to know what your thoughts are and what your next steps would be if you were in my position. We have two frosties and I want to make sure I leave no stone unturned before considering transferring them.
What are the ideal E2 and P4 ranges after a positive beta in a DE or FET cycle? My E2 was over 2300 at 12dp5dt and then over 2600 15dp5dt. P4 was 46. For some reason I feel the E2 was high.

First, I wanted to say that I'm very sorry for your losses. Big ((HUGS)) to you.
Not to minimize anyone else's failures, but I honestly feel that DE failures are a special kind of hell, especially after all of the emotional and finanicial
Me: AMA, DOR, undetectable AMH, carrier of SMA, MTHFR homozygous C677T, high cytokines, low IGg B cells, Factor XIII V34l mutation, High Anti-Phosphatidylethanolmine, borderline hypothyroid and mildly insulin resistant.
MH: No known issues, aside from MTHFR.
Recommendations from RI: LIT, Humira, IVIG, Lovenox, Baby Aspirin, Folic Acid, Low-dose synthroid and Metformin.
Currently taking: Pre-Natal vitamins, Foltanx, Low-dose synthroid and Metformin.
When cycling also taking: Baby Aspirin, Lovenox, Humira and IVIG.
TTC since 7/11/09..with medical intervention starting in 2010, including 4 failed IVF's and 6 failed IUI's...only "success" was a chemical pregnancy in 11/2012 (credited to use of DE).
FET #1 (11th cycle) on 8/23/13 = BFN.
FET #2 (12th cycle) Transfer of last two (day 6) donor egg blasts on 1/22/14 = BFP, EDD = 10/10/14 (please stick little BOY!)

Met with RE 4/11. 2 IUI's BFN. DE best option. Switched clinics to do "shared" program. Had to retake all tests and a mamm that put me behind and then on a DE waiting list for 12 months. Picked a donor!! (10/13/13) Got matched. Estimated transfer in December. After 2.5 years of patiently waiting I will finally cycle....can hardly believe it. DE cycle got cancelled. One of her tests came back positive. Waiting for another donor. Donor picked!! (1/18/14)

I'm so sorry for you loss and understand your quest to find answers before you move forward with transferring your remaining frosties. I would probably seek a second or third opinion until you feel comfortable to decide to seek treatment or roll
