Diaper rash..

DS has a horrible diaper rash right now. He has a slight cold, runny nose, no cough or fever. I think he is teething. But his poor little butt is horribly red with raised bumps. We put A&D ointment on him at every change like we have for months, and he has never had a diaper rash like this before. Poor thing just screams when we change him. I'm not sure what it is coming from, other then teething possibly. Any suggestions on what to do to soothe the poor bottom?
