I'm sure this has been posted about, but could not find a recent post. Have you started your birth plan yet? FTM, anything that surprised you on the birth plan template via the bump? At my doctors appointment last week I was handed third trimester paperwork/checklist. I'm a bit overwhelmed to say the least.
Re: Have you started your birth plan?
Ditto. If you find a template that says exactly this, I'll take it

Yeah, my birth plan the first time was "yes to drugs and please help me have a healthy baby however that needs to happen". And it worked, although it ended in a c-section.
I'm having another c-section this time, so I won't have a plan at all besides to allow the baby to do skin to skin with my husband while I'm being stitched up, if that's possible.
No birth plan.
We do have to fill out paperwork for the hospital though and get pre-admitted. Depending on which hospital we pick we also have to have a "pre admittance interview" if we choose one of them. I kind of don't want to pick that one just because of that.
I have mine broken into parts 1st is introduction ie., who will be in room, please be sure no one is allowed in my room except for labor support, I plan a birth without the use of pain medication...
then wishes or fears DH has a lot ie., no induction, no pain meds...unless I ask, the ability to labor as I wish, walking, tub, labor bar etc..., wear what I want, prefer to tear vs epsiotomy...play music
After delivery, delayed cord clamping, skin to skin, if my baby needs medical treatment my husband is to accompany baby at all times, breastfeeding, unless medically necessary I do not wish to have bottles given etc...
I also included in case of a CS area with my wishes...
this is only a little bit of what's important to me during labor and delivery...and I am very open to "subject to change" should myself or baby be in harms way...it is good to have one, GL putting yours together

My birth plan: "Healthy Mama and Healthy Baby".
I am willing to roll with whats best for us.
That's my plan! Oh and hopefully this baby flips so I don't need a c-section.
This is my third and my brith plan is get the baby out safely and healthy. After my other two I have lower expectations than most.
Keep a list of things you NEED to happen (no epi, no pit for example) and things youre flexible on like insertion of IV fluids and or laboring in a tub vs the bed.
It all depends on what you want out of your birth.
My MW told me not to write anything ridiculous like "forceps only if necessary". ....as if they use them at a whim and because they fancy doing so. She said just be sensible about what you write. My friend just had a c section and she told me to write down for consultant and not the trainee consultant to perform this procedure. If you don't state this they don't ask. At least that's how it is at my hospital with it being connected to the university here.
We had to do ours for Bradley class last week. The key things are to be deferential and polite, not demanding, and to understand that the ultimate goal is healthy mommy, healthy baby. Since we are trying for a natural, med-free birth, we put in things like we are first time parents and would appreciate suggestions as to positions and non-medical techniques for pain relief and comfort as well as to induce or speed up labor as needed, do not ask us if we want any medication for pain relief, we want to labor using the tub and birthing ball as well as walking around and changing position, and that we do not want to have my water broken until I am fully dialated. I also said that while we want to avoid any medical intervention, I am willing to have them put in a saline lock. And in the event that a C-section is necessary, I want double layered suturing in order to allow for best chances for a VBAC with future pregnancies.
For the immediately after birth section we put that we would like immediate skin-to-skin and delayed cord clamping, DH wants to cut the cord, and we want to delay any measuring/weighing/Vitimain K/eye ointment for at least one hour to give us a chance to bond and attempt to nurse. I also put in that I would like to try nursing and fundal massage to avoid using pitocin for placental delivery. We also put in that we are planning on exclusively nursing and would therefore like a consultation with the LC as soon as possible, and to avoid giving anything to the baby (pacifier, bottles).
I still have some work that needs to be done on it, but I hope this gives you some ideas of what to include.
