My engagement band came from Israel Rose. Seems like people love them or hate them. We had a very good experience. It's an estate ring from the 1940s. They made it a little hard to tell which are antique and which are replicas. But I LOVE my ring. I would probably use another antique ring dealer if I could do it again, but only if I could get my exact ring again!
We had my band made locally to replicate the matching band that would have come with the ring originally.
DH had both rings custom made from a jeweler here in Tampa. I suggest looking into custom work (it doesn't have to be crazy expensive) from local jewelers, or a vintage ring, if you want something uniquely yours.
We looked at Costco before deciding on a local jeweler who makes all his own rings. The price was about the same as regular jewelry store but the quality is outstanding. Might try finding one of those guys close to you.
Both of our rings came from Jared. I lost one of my stones a year later (hit it on a rock swimming in Jamaica) and they replaced it for free. Plus they do all of our cleanings and re-dip my bands since they are white gold once a year for free.
I also highly recommend Blue Nile. I have a pair of diamond studs and an anniversary band from them. They have decent prices and excellent customer service.

We had previously found their online eBay store and then realized they were located right where we were going to be traveling in the next few weeks. It was a little nervewrecking buying a ring in a wholesale atmosphere, but I couldn't be more pleased with how my ring turned out. It truly is a 1-of-a-kind creation.
We had it appraised back home and it came in DOUBLE what we paid for it. Can't beat that!
We have a jeweler that we go to for all of our family jewelery...I would go in person to look. But there are a lot of different sites out there to browse. Blue Nile is one I can think of off the top of my head.
H had mine custom made at a small jeweler. I think that's really the way to go. Prices and quality are usually better than a lot of chain jewelers, IMO.
Mine has 2 bands soldered together on the top and bottom.
A quick note about appraisals. Your ring should appraise for double or more. An appraiser gives a value to be insured for so it's based on future value. If it doesn't appraise for double, you're probably paying too much for the ring.
My DH used Good Old Gold to find the diamond, and had it set in a Tacori band. It was pretty cool, once it was put together they sent him a link to a video to show what it looked like sparkling under different types of light.
