DS is sick again. He has had a fever between 100102 for 30 hours now but it will go back to 98 for awhile after I give him Ibuprofen. I was thinking that the rule was you should take them in if they are still running a fever after 24 hours on meds. What general rule do you follow?

For me If they have a fever for 2 full days (so 48 hrs) I call the pedi and request an appt. I don't medicate fevers unless it hits 101, or it's bedtime.

I would call. Last time I called it was four days with a fever over when not controlled by medication., but I would call your pedi to make sure.
As long as they are still active and staying hydrated, they can run a low grade fever for a few days without seeing the pedi. I try to medicate only if the fever is over 102 or they seem uncomfortable because fever has physiologic benefits. It sounds like the medicine is working, just maybe not the 6-8 hr duration. I will still alternate motrin and tylenol because it seems to work better overall. If the fever persists after 4 days, or if they become lethargic or dehydrated, I would definitely try to get the LO evaluated. Before that, there's often not that much that they can tell you about the etiology. After a few days, rashes and other tell-tale symptoms may appear to help provide a diagnosis. I've talked to our pedi's office on several occasions regarding when to bring them in and every time they tell me to wait for several more days as long as they are feeling ok. Sure enough on day 3 or 4 the fever breaks and it ends up being some common childhood viral disease. I hope little man feels better soon.
