OHSS vs Progesterone S/E

Hi All,

Hoping someone can help me on this board.

I had my e/t on 2/11 with 23 follies, 19 mature, 9 fertilized with ICSI, and had my e/t of 2 embies on 2/16. I had mild bloating right after retrieval, which resolved on its own, and have felt great up until the last few days. 

My abdomen is very distended, I have been measuring it daily and have had a 2" increase in size around my belly button but for me, I look about 6 months pregnant! I've gained 4 lbs total since the E/R. The bloating gets worse after eating a meal and I've noticed I have terrible stomach pain when eating, and have been taking Tums like crazy to help with the gas pains. I also notice that I'm taking shallow breaths and can't lay down to sleep. It hurts to laugh, sneeze, etc. d/t the abdominal pressure and the pressure also radiates up towards the top of my chest.

I don't want to go to the ER for fear that they will do an abdominal Xray and I'm wondering if I'm pregnant (got a clear + today on a HPT). I know OHSS can develop late and hit hard if you're pregnant. I have read that progesterone can cause gas pain and indigestion and don't want to make a huge deal of this if it's just the hormones!

Any thoughts? I'll call my doc in the AM no matter what but am wondering if I should be more concerned and call ASAP.

  • It sounds like OHSS :/ You likely have fluid build up in your abdomen, and it can creep up into your chest making it hard to breathe. Oftentimes, the fluid needs to be drained if it becomes severe. Does your clinic have an after-hours nurse line or a physician on call line? I would call them right away. In the meantime, chug gatorade like it's going out of style and eat lotsa high protein. GL!

    **TTC #1 Sep 2009-Feb 2013**   Me: 33 DH: 32 (healthy, SA = normal)

    DX (May 2010) = Lap #1 showed Stage I-II endometriosis. HSG normal. Tx with 6 mo Lupron (which did nothing...) Lap #2 June 2011 to remove endo, uterine fibroid, and polyp

    IUI #1-#3 (July-Sep 2011): All Clomid 50 mg + unmonitored IUI = All BFN

    October 2011 - April 2012: Outta $$$ (no insurance coverage) = back to tryin' on our own

    May 2012 - August 2012: Mental break from trying

    October 2012: New OB. Bloodwork normal aside from slightly low progesterone

    November 2012: IUI #4: Letrozole 2.5 mg + Novarel trigger + monitored IUI + progesterone = BFN

    IVF #1: Long Lupron Protocol,Menopur,Follistim, ER = 2/12/13 27R/26M/23F

    2/17/13 Transferred 1 perfect Grade 1 blast. 17 frostbabies.

    HPT 7dp5dt: BFP!!! Beta #1 9dp5dt: 108 Beta #2 11dp5dt: 368

    U/S #1 3/21: Heartbeat and one perfect bean!!  DS born 11/6/13 via c-section


    TTC #2 March - August 2015 = all BFNs

    Prep for FET #1: Aug 2015: found "mass" in uterus on saline sonogram; scheduled a hysteroscopy to remove. 

    Sept 2015: Hysteroscopy: mass turned out to be scar tissue(?), all removed. Recheck saline sonogram 2 weeks later showed "stuff" still in uterus (more scar tissue? remnants from surgery? Will see if clears with a period). Recheck saline sonogram 1 week later: all clear! Start Lupron and Estrace. Follow up u/s 2 weeks later: uterine lining only 5mm, add in patches. Follow up u/s 1 week later: uterine lining = 4.3mm (argh!), stop estrace and patches and start estradiol injections twice/week. Follow up u/s 2 weeks later: uterine lining = 5mm :/ Instructed to keep on estradiol injections and recheck in 5 days = lining still at 5mm. Need to make a decision - go for it? Or toss in the towel w/ this cycle and start all over again next year with a new protocol. Decided to go for it as we had 17 frosties; what have we got to lose?? 

    12/2/15 = transferred 1 perfect Grade B blast

    12/9/15 = HPT - BFP!!!!

    Beta #1 12/11/15: 236    Beta #2  12/14/15: 503   Please stick little one!!!!

    U/S scheduled for week of 1/4/16

    Oh, yuck!!! I have to 3-12 hour shifts in the next 3 days and was really hoping not to miss work tomorrow. I'm just going to feel so silly if they say everything is ok, and am terrified of getting any treatment with 2 maybe babies growing in there! 

    Cypress-thanks for responding to me, looks like we're cycle buddies! I'm jealous you're getting your beta in 2 days, they're making us wait forever it feels like.

  • The way you are feeling is not silly at all. It's pretty serious. So please call and get some help!! I am at high risk for OHSS as well. Avoided it with my ER thankfully, but now am nervous (got a BFP on FRER this morning), so...trying to do everything I can to avoid it. Doc said it would present exactly as you are describing...I would look 7 mo pregnant, have a lot of abdominal discomfort, possibly difficulty breathing. Some women on this board have had to go in for multiple abdominal taps. I hope you don't have to go that route. Just take it easy...the more you move around the more it can get aggravated. You may have to call in sick - you need to do what's best for you and those lil' embies!! If you have to go to the ER, just tell them you are an IVF patient and that you may be prego. DON'T let them do a pelvic exam on you. Have them get in touch w/ your clinic and they can guide them.     I had moderate OHSS. It was extremely painful and I had some of the symptoms you're describing. RE was most concerned about me gaining more that 3lbs in one day and twisting an ovary. If you decide to go to the ER, tell them you want a u/s. I hope you feel better soon. GL

  • I feel like this, i am super bloated and still a bit achy from my ER that was 4 1/2 days ago. I have had a racing heartbeat at times, warm body temperature and constipation.

  • I had mild OHSS when I did my injectable cycle and it sounds just like what you're saying. I know once you're pg it does usually get worse so I would def contact your RE and get it checked out.  Drinks lots and lots of Gatorade and just get to the docs...feel better :(

    I was confused about high sodium foods, because a nurse told me NO but then I saw it all the time on Bump for OHSS. I confirmed with one other nurse AND the doc that said "NO HIGH SODIUM DIET FOR OHSS". They said it would make it WORSE.  So...FWIW. I just stuck to gatorade and high protein.

    I was told to follow a high sodium diet too and I go to one of the top centers in the USA. Maybe it's patient specific...obviously it wouldn't be good for someone with high blood pressure to be on a high sodium diet.
