PDA and indocin

Our 24 weeker who is now 27 weeks...was found to have a heart murmur since she is now off the ossulator vent. They did a echo today and she has a large PDA. Does anyone have any input or advice or stories to share? Labs r being done and if all is good she will start on indocin every 12 hours for 3 doses

  • Our Anna (26 weeker) had a PDA. She couldn't have the meds due to her brain bleeds but thankfully it resolved on its own. I don't remember if they said it was "small" or "large," though.

    Do u remember the reasoning why she couldn't get meds due to the bleed? Our little nugget has a bleed too but r still using meds for the PDA ...the dr is checking her platelet level first tho before giving

  • DD was a 24 weeker (well, almost), and they started her on indocin, but she developed NEC so she ended up needing the surgery. I think indocin is usually supposed to work well, but the surgery, while kind of scary, wasn't that big a deal in the end.

  • One of my twins (28 weekers) had 2 doses of Indocin for a moderate PDA.  He was symptomatic from it (increasing 02 needs, brady/desats).  It was able to close smaller but didn't fully resolve until later on but he was not symptomatic at that point any more.  I have heard some where that it's really most effective in the first week or so of life?  Otherwise PDA ligation is an option if the meds don't work.

    She has a grade IV bleed on the right, into the brain matter. They said that the medicine constricts all blood vessels in the body and that could have aggravated the bleed and caused it to spread further.

  • All four babies did indocin and it only closed one PDA. The other three babies had the PDA surgery. The babies did so much better after they had that PDA closed. It was amazing to see improvement pretty quickly. The idea of surgery can be scary but the PDA surgery is very common and a pretty basic procedure. Thoughts to your little one!

  • My former 26 weeker also couldn't take meds due to the IVH he had. He had to have surgery which actually took 10-15 minutes. It was still pretty scary considering that he was probably about a week old at that point. They monitored his heart along with everything else very closely after that and he had no more problems. Thoughts and prayers to your little one!

  • Indocin rarely works on babies that are 3 weeks or older. I don't know why but I read it in a preemie book.

  • Indocin didn't work for our 24 weeker but like pp's said, the PDA ligation did and he really started improving overall and gaining weight after the surgery. It was super scary at the time but we were also reassured that it is a very common procedure and the surgeons had done lots and lots of them.

  • My 25 weeker developed a PDA at 1.5 weeks old.  It was very large - large enough that there was no murmor because it was wide open.  They discovered it when doing a workup because she was maxing out vent settings.  Luckily her PDA closed after a 2nd course of indomethacin.  It did reopen and she had a mild-moderate PDA that we watched from about 32weeks-39weeks adjusted.  It self closed before there was a need for surgery.

     Similar to the above story, my son was given indocin and didn't respond to it.  We elected to have his pda closed surgically at 4 days old (26weeks gestation).  He was still on the high frequency vent.  I honestly think us not waiting to close his pda helped him.  Today, at 1O yrs old, he has a long scar that runs across his left side.  It healed very well and you can't really see it unless you know it is there.  Good luck and prayers for your LO. 

  • My twins both had the PDA, DD's close after 3 rounds of indocin. DS's didn't and he needed the PDA ligation. It was so scary having my 2 lb baby going into surgery, but the doctor reassured us that it was a routine procedure. The surgery was really quick!
