Medical Power of Attny?

Hi Ladies,

Since we are apparently going to have a baby pretty soon (maybe sooner than we think, according to the OB), I'm wanting to get some sort of medical document written up to authorize DW to make decisions for our little baby in case I'm out of commission. We have our marriage cert., which helps, but our pediatrician would like some notarized document that grants DW authority to make medical decisions, though she is fully supportive, etc.

Any thoughts or example texts would be much appreciated. I know, of course, that these things are always only as good as the people who will observe them, but we want to do what we can.

Ours were done by our attorney, however I believe the hospital where delivery will be should have a sample form or even a fill in the blank form.

I would try to Google and some documents will come up that you can change to what you want/need. I live in Ohio and I know that you can go to most banks and they will notarize for free.

Since we planned to do a second parent adoption, though not sure of the laws in your state, our attorney was willing to write something up until the second parent adoption went into effect.

I've used the forms at the hospital before. They have the advantage of definitely being valid in your state.

We had our lawyer draw up some paperwork before the twins were born because I was worried about the same situation. Also our pediatrician had forms on file that we just filled out and got notarized before the boys were born. But I'm sure the others are right that the hospital itself might be able to provide you with the appropriate documents. Best of luck!

I would agree that you should first call the hospital and ask what documentation they would need should something ever happen. If they don't have a specific form, you can check your local court website for forms. As for having it notarized, most banks provide this for free or little cost if you are a customer.

Our hospital had forms for us. We asked for them when we checked in and they sent the Chaplin in to witness the forms.

They then put one in my chart an made us like 5 copies we could keep different places. We keep one in the diaper bag, gave one to the pediatrician etc.

I am from CT. I was told that within CT my wife will be able sign the birth certificate but we have been encouraged to look in to legal adoption so she would have rights. If we were going to another state and something came up she would have legal rights. This is very interesting and I will be checking with my doctors as well if this is something that I should be looking in to as well.
