Hi Ladies,
Since we are apparently going to have a baby pretty soon (maybe sooner than we think, according to the OB), I'm wanting to get some sort of medical document written up to authorize DW to make decisions for our little baby in case I'm out of commission. We have our marriage cert., which helps, but our pediatrician would like some notarized document that grants DW authority to make medical decisions, though she is fully supportive, etc.
Any thoughts or example texts would be much appreciated. I know, of course, that these things are always only as good as the people who will observe them, but we want to do what we can.

I would agree that you should first call the hospital and ask what documentation they would need should something ever happen. If they don't have a specific form, you can check your local court website for forms. As for having it notarized, most banks provide this for free or little cost if you are a customer.
They then put one in my chart an made us like 5 copies we could keep different places. We keep one in the diaper bag, gave one to the pediatrician etc.

Jacob and Samuel born 1/29/13 at 36 weeks.
I am from CT. I was told that within CT my wife will be able sign the birth certificate but we have been encouraged to look in to legal adoption so she would have rights. If we were going to another state and something came up she would have legal rights. This is very interesting and I will be checking with my doctors as well if this is something that I should be looking in to as well.

New Plan: Reciprocal IVF, me as carrier wife's eggs. Just went through insurance and received partial approval, so my part will go through my IVF benefits and wife's part will be out of pocket. Now just finalizing finance plans to cover the oop costs. Doctors office is in process of moving to a new building so there are no IVF start ups until March/April 2015.
