  I did some research into this, as well as spoke with the neos, and I found that it is recommended to do the vaccines using actual age.  Adalyn had her 2 month shots while at the hospital.  They held off on the Hep B until she had her 2 month shot (since it was part of a combo shot) so that she didn't have to have an unnecessary stab.  She was 34 weeks gestation/8 weeks actual when she had her first set.  She has had all of her vaccines since then on the actual age as recommended by the CDC.

  We did them at a semi actual age I guess. My twins were born at 26 wks, and I think I started shots at 2.5 mo bc like you, I hadn't expected it so I needed time to look into it..we gave all their two mo shots spread out a week apart.  I however waited on their hep b until 4 mo.  I had found that hep b shouldn't be given before 4.4 pounds. If I remember...I did dtap first, then the next week hib, the next week prevnar. Then at their four mo, I did pedarix which has the tdap with their first hep b. good luck. If you choose your own schedule, be prepared to stand your ground. The nurse practitioners were perfectly fine with us spacing, but I had one nurse stand outside our door during report time tell the next nurse basically how dumb it was that we weren't just giving all the shots at once.
