First, I've never heard anyone call it a pack back. It's cute.
I don't like using backpacks or messenger bags so I use a regular tote style diaper bag or large purse without any issues.
LOL please excuse the misspell on this post. I'm running on 4 hours of sleep, I had no idea that I wrote pack back until you pointed it out. Now I understand why my spell checker thought I missed typed something.
I have a Jujube Packabe that I can carry as either a backpack or a messenger. I loved it, particularly when the girls were little -- I could put it on my back, wear DD2 on my front, and still have a hand free to hold DD1's hand. When DD2 got bigger, I wore her on my back and carried the bag as a messenger.
I love the bag, it has held up very well, and although I mostly use it now for travel or long day outings, it's still got probably years of wear on it. I had one issue with a clip breaking and Jujube replaced the entire bag even though it was beyond the warranty period -- they were very responsive and I appreciated the level of customer service.
Plus they have lots of fun patterns, or you can choose something solid and less flashy.
I had both and honestly, as long as I was hands free with my kids, I didn't care. I needed both hands - my girls are 21 months apart. I used a back pack for the most part just because it was bigger and I was carrying around diapers and change of clothes for 2 plus the bottles, snacks etc and that can get bulky. My purse now (no diaper bag as kids are older) is a messenger bag style and I love it! With the back pack, I did make sure to always wear both straps and watched the weight as carrying the baby and the bag can get heavy and I have back issues.
I have yet to find the perfect diaper bag. In general, I'm a fan of smaller. With two kids, I like messenger style so I'm more hands and arm/shoulder free. But the messenger is tricky when DS2 is in the Ergo, which is a lot.
I agree with Scout that multiple bags are needed for different types of outings.
