Pregnant after IF

More bleeding... (long)

Since the 13th of this month (a little less than 2.5 weeks), I have had 9 episodes of bright red bleeding (well 2 have had a bit of brownish mixed in).  Each episode is a gush of a LOT of blood.  It's like that terrible start of your period when you go the bathroom (sometimes worse).  I went to the Dr on the 14th.  He did an u/s and thought the bleeding was due to a cluster of vessels on my cervix.

I had 4 more episodes after that Dr visit and wasn't convinced this was a normal presentation of a cluster of vessels spontaneously bleeding.  Up until yesterday, all have been in the middle of night.  I went in and saw a different Dr in the practice.  He saw what the first Dr saw, but from his exam he felt that the blood was coming from the uterus.  Placentas looked good with no clots and babies were moving with good heartbeats.  He did say that he couldn't visualize the very tip of one of the placentas because my bladder wasn't full.  Well don't make me give a urine sample when I walk in the office!  He did not really have answer for the bleeding.  He was not concerned and even said we could have sex.  The plan is for me to document when it happens, call if it is persistent and I'll see him in 2 weeks.

I am a clinician that works for the same company (clinical dietitian) so I have full access to my chart.  In his note he referred to the "cluster of vessels" as a "clot on the cervix."  Just different terminology from the docs. He said the results of the u/s were unremarkable.  Then he said "possible marginal abruption based on history." I'm assuming by history he means my description of the episodic bleeds.  A marginal abruption seems like a big deal to me from my research.  Now I'm freaking out.  I have had 3 more episodes in the past 24 hrs.  2 of those have been reddish brown.  I'm chalking this up to irritation from the exam yesterday.  I thought dealing with baby B's omphalocele and possible issues associated with that were enough.  I just can't handle this!  Any advice? Anyone deal with this?

  • Sorry your going through this with little explanation. I would switch OB n be off my feet with plenty of water till I get detailed explanation. TP the bleeding stops n for healthy babies.

  • I am sorry this is happening to you, and it's really stressful that you can't get a clear answer. FWIW, it is possible to have a partial abruption that isn't always visible (that is, it is always there, but based on the skill of the person doing the ultrasound and positioning, etc. it might appear on some scans and not others). Are you seeing a MFM or a regular OB? I think if you aren't seeing a MFM, you are very justified in asking for a referral to one at this point. And either way, definitely alert your current OB that you are still bleeding regularly. If it is anything more than spotting, I think it is sort of unlikely to be the cervix, but I am not a doctor :) It is definitely something that should be closely monitored, though.

    I had a partial abruption with my twin pregnancy and it was taken seriously and I was on full bedrest. I am not trying to scare you since that is not necessarily what you have (and even if it is, many people do have a 2nd tri partial abruption and go on to have a successful pregnancy). So I guess my main advice is to just be persistent. IMO it is not "nothing" if it keeps happening. Thinking of you and hoping you will get more clarity soon.

  • I am seeing a MFM but they work on consult basis, they are not my primary.  I am seeing the MFM however for the baby's omphalocele and not for the bleeding issues.  I have an appt with them in 2.5 weeks for u/s and amnio.  I will have to switch physicians no matter what because I have to deliver these babies in a facility with a NICU.  The practice I'm currently with only delivers at a hospital 45 min from the hospital with the NICU.  

     I'm concerned about switching Drs now because I really like my MFM and want to continue with amnio and all the omphalocele work up with him.  The practice I plan to switch to has their own MFM, but I would rather stick with mine.  At this point, I just don't know what to do and what physician I should be seeing.  I'm working from home on the couch today and plan to take it easy the rest of the work week and put myself on bed rest in the evenings and on the weekend.  If the bleeding doesn't stop or improve, I'll have to take some sort of action.  I just feel like the crazy patient that has called 1000 times.  And I was the b**ch that refused to see a midwife and demanded a physician. I'm also not understanding why he wasn't concerned and even gave me the OK for sex.  Thanks for the support ladies!

  • Is it possible to call the MFM's office directly and explain your situation and ask if you can be seen sooner than in 2.5 weeks? It just sounds to me like maybe this is something for them to handle if it is an ongoing problem and your OB has no insight. Thinking of you and hoping today was less eventful.

  • Hi, I'm new to this board, but my wife and I are going through a similar experience. We too dealt with unexplained infertility and after 9 failed IUIs, finally became pregnant with twins in November with IVF. We were completely thrilled and everything was fine until our MFM ultrasound this past Monday at 12w3d. The doctor detected a ventral wall defect on Baby B and his/her intestines at least are outside of the body. Baby A is normal. At this point they are saying it is unclear if it is an omphalacele or gastroschisis. The insertion point of the umbilical cord is not visible. They told us they won't know definitively until 18wks. As you know, we were overwhelmed with fear for both of our babies. We were immediately brought to the genetic counselor who helped us process what just happened, explained again what it all meant, and reviewed our options. She was outstanding. Obviously the most concerning is the chance of a chromosomal abnormality and other defects if it's an omphalacele. Since we are in our 13th week, we could still do the CVS and we felt that was the best choice for us. We had the test yesterday and should get the initial results for Trisomy 13,18,21 and gender mutations tomorrow. The waiting and the unknown is difficult and scary. Fortunately K has not experienced any bleeding, even after the procedure yesterday. I am so sorry that is happening to you, because I know it must just exacerbate what is already such a frightening situation. I think you are doing the right things by seeing a different doctor when you're just not convinced it's ok. Also, I don't think you should feel bad for continuing to call when you are still having the problem. If it is truly not concerning to them, then it is their job to explain it to you in a way that is reassuring and gives you confidence in their judgement. We're fortunate to live near Boston and so far I can't say enough positive things about our level of care. Our OB's policy for a twin pregnancy is to only be seen by physicians (no midwives, NPs, or PAs). We were immediately referred to the MFM as well and went first for a nuchuel test. It was at this test that the defect was discovered. We will have to leave this MFM though, because while the hospital we had chosen does have a NICU, they do not do pediatric surgery. We will transfer care to Childrens Boston and delivery in a hospital close by. I don't know where you live, but I would look at your options and find the hospital closest to you that has the most experience with omphalacele. The distance might be greater, but having an experienced team is worth it. Although I hate that anyone else has to go through this, it's nice to know we're not alone.
