I just got back from visiting a friend who just had her second baby, her first is 2 yrs old. She is a SAHM now and I'm amazed at how hard she works, non-stop, all day long with those two boys. I'd like to send her a little something to make her feel appreciated and special. Her husband is pretty awesome too, he works hard outside the home and then is very involved when he gets home. So, I wouldn't mind getting something the both of them could use.
Any suggestions?

Food. If you can't make it yourself, then a gift card to a local restaurant. Or maybe a housecleaning service.
Also, if you know several of her friends/family, you can set up something on a website like carecalendar.org. People can sign up on specific days to drop off a meal or help with an errand, etc. We've got a wave of 2nd babies arriving in my DD's playgroup and we've been using carecalendar to arrange meal drop-offs. It's worked out great for everyone.

I second the above suggestion! Food is always appreciated and saves them time & energy. A nice big casserole or lasagna usually works well.
Otherwise, I'd send her a gift card for a pedicure or massage, with an offer to babysit the boys so she can go have it done.

Food it is! I think I'll get them a gift card to a place they can either order delivery or go into the restaurant. Unfortunately I live across the country from them so can't babysit. :-(
