High-Risk Pregnancy

Diminished Diastolic Umbilical Flow?

Hi all- I intro'd a while back- I will be 35 wks tomorrow.  I have GD (take glyburide 2.5 mg twice a day), hypothyroidism, and Lupus- My baby has complete heart block. 

 I had my weekly Biophysical Profile today and it came back with some surprising findings... I am waiting for my MFM to call to discuss but wondering if you ladies had any knowledge/experience with them

1. they said my placenta is a grade 3 which the tech said was "mature" and is usually seen at term

 2.  They said that there was diminished diastolic flow of the Umbilical Artery (the tech seemed slightly concerned but didn't say to what degree

3.  And my amniotic fluid was abnormally high but the report I have doesn't list the AFI # so not sure...

 I am resisting Dr. google at this point but my willpower is only so strong- any ladies have any knowledge on these findings?

  • I am sorry you got some not so great news.  You have made it to 34 weeks which is pretty good, are they talking about taking the baby early?  I would think this is a conversation that might be happening because he/she might do better out here than in there.  I pray you don't need to have that conversation and that things stay stable.   • I really don't know much about those things...I do know my parents was a grade 3 when my son was born. I hope you find some answers soon...34 weeks is pretty good if you r LO comes early.

  • Thanks ladies- I know my baby is at a good GA if he needed to be delivered- he has always been in the 90th percentile or higher for weight and growth measurements :)  I know that doesn't equal lung maturity though!

    My MFM is going to take the conservative approach and is sending me for BPP's 2-3 times weekly to monitor baby's well being more closely-  Any signs baby is in distress or anything and he would be out.  He said typically in GD mom's they do see a placenta that matures more quickly and that at this point it doesn't appear to be having an effect on baby.  The cord flow issue he said is a little more complicated as it is typically seen in IUGR babies and my LO has been on the opposite end of the spectrum.  They will repeat the study at my next BPP and if it is still an issue then we will discuss options- basically he said it is a very early sign that the there is distress but in non IUGR babies there is high incidence of false positives.  Same with the fluid issue- could be a one time scan thing and will address if continues on follow up scan
