Introducing Cole Philip! (birth story with 48 hour induction, complications, CS) PIP

Copied from my other BMB (but you guys will always be my first!) Cole Philip arrived at 9:14pm on January 12th. I was 41 weeks and 1 day pregnant, attempting a VBAC after having a CS with my son 17 months ago. I will try and share my birth story (as much as I remember ... the timeline has already blurred). I was admitted for induction Thursday night. They wouldnt let me wait an longer than 41 weeks for the VBAC. They started me with a Foley balloon at 9pm. I was 1-2cm dilated to start and was already showing some contractions on the monitor on admit. I had painful contractions throughout Thursday night, but when they removed the balloon at 1pm Friday, progress had not been made. Friday at 1pm, they inserted a second Foley with higher balloon pressures. They also started low-dose pitocin. I walked around A LOT and even used the yoga ball. Contractions were painful. They took out the balloon at some point. (They rest of this is already jumbled in my head.) I believe that I had progressed to 5 or 6cm, but he was not dropping at all still -3 position. They increased the pitocin by quite a bit at this point. At 3am Saturday (day and half into labor), I got an epidural. I had a bad experience with Mason, but ths worked well. I was able to get a few hours of rest finally. They encouraged the epi because even though I was contracting strongly, baby would not drop. The were hoping this would relax my pelvis and help baby drop. Throughout everything, I was having fevers. They started off 99 but persistent. Then I started spiking to 101.6F. At that point, we were also having issues with Cole's heartrate and abnormal reactivity to contractions. This was Saturday afternoon. They gave me IV antibiotics and stopped the pitocin for a while. Around this time, they broke my water. There was evidence of meconium, although it wasn't really extreme. They just said that the NICU would have to be present for the birth. When Cole's heart started behaving again with the pitocin off, they decided to give it another chance at a low dose. Once again, I was in some extreme pain. The epidural wasn't quite cutting it, but tried not to complain. At my final check, I was 8cm dilated and -1 position (still high). They said my cervix was getting very swollen and irritated as well. My fever wouldn't decrease with IV Tyenol (had never even heard of ths as a nurse) and multiple antibiotcs. It was time choose a Csection. They were amazing about supporting the VBAC and still even left the CS up to me. I felt completely ok about the decision at that point. Almost relieved because I was starting to worry about Cole. I had a full 48 hour labor so I can't say that I didnt try my hardest. The CS went great. A much better experience this time around. A positive, happy experience. I will remember it every time I hear Buffalo Soldier (anesthisiologist's background music). Recovery has been great so far (about 20 hours since surgery). Minimal pain. I only took pain med once after my first time up out of bed overnight. I am in and out of bed myself, walking the halls alone. Cole is perfect in every way. He even nurses well already. He had his circumcision this morning. We both need antibiotics for 48 hours, but no more fever and no specific infection source. Daddy stayed overnight, but he went home to our son. Mason met his brother today, and it went well. I am now on my own for the night. It is a bit if a challenge with rooming in because I am so slow in and out of bed to bassinet, but it is manageable. Going to eat some dinner and rest up before a few more evening visitors. Hope you enjoyed my story. image

    We use IV acetaminophen a fair amt at our hospital for people who are NPO. It's called Ofirmev. It's expensive but pretty effective. 

    What I have not heard of is using Pitocin so extensively for a VBAC. At our hospital they apparently don't use it at all for VBAC's!? Guess different docs have different practices.

  • Congrats to you and your new family of 4!  Cole is precious.  I'm glad everything turned out OK, even if not what you originally had hoped for.  You sure gave it a good try!  My labor with Andrew was 32 hrs, I can't imagine longer like you endured!

    I've been researching VBACs for our imaginary next baby. Everything I've read has said that doctors don't use Pitocin and rarely induce for VBACs. Of course, I'm not a doctor (nor do I play one on TV), but I thought it was interesting.

    Anyway, CONGRATS! Cole is such a cutie and I love his name. H & H recovery! Smile

  • Thanks for all of the congrats! We are doing very well. We both just got our final dose of IV antibiotics. Cole will be taking liquid antibiotics prophylactically for hydronephrosis until a follow up ultrasound of his kidney in 4-6 weeks. He is nursing well and is a super duper pooper. I am in love! Mason did well with hs first visit, and he will be comn back in this afternoon. I miss him so much. As far as inductions for VBACs, there is a lot of variance between medical providers. Many will not consider any induction for VBAC. Some will only use non-pharmacological methods, like breaking water and Foley balloon. Pitocin is approved for VBAC induction, but it is typically used in smaller doses, and you must be very closely monitored. I started off at a very low dose, but they did end up increasing it a decent amount. The first doctor I had during the long induction was very pitocin conservative, but the last one I had shared a lot of research with me about how it really wasn't contraindicated in VBACs. The medications that are contraindicated in VBACs are the prostaglandins Cervadil and Cytotec. They should never be used in VBAC inductions.
