Injectables? WWYD?

We've had 2 IUI's with Femera and Trigger, is it too soon to try injectables!? The nurse said "the use of injections and increasing the number of eggs will definitely put you at risk for multiples.  If you get pregnant (25% chance), you have 20% chance it will be multiples" Injectables would cost us give or take about $500 each cycle adding that to the $750 vial of swimmers, $100 shipment of swimmers and $600 in clinic fees. WWYD?

  • This is such a hard questions and one really only you and your partner can determine for yourselves. I know families who have gone straight to injectables due to age/other fertility factors/monitoring cycles and I know families (mine included) who would have needed to work toward it with great caution. For us, avoiding multiples was a huge priority as they run in my family, I work with twin families a lot and know what it takes and it would have been a huge financial impact. If it had happened, we would have dealt and loved them both, but we were also concerned until our first u/s showed just one. All that said, I would definitely have considered it if I was facing injectables or IVF. I would discuss the probability and your comfort with having twins, maybe read a book about parenting twins and/or premies (multiples increases that likelihood as well) and I'm sure others will chime in who have chosen injectables and/or have twins.   • After my poor response to Clomid, I was given the choice of injectables + IUI or IVF.  We chose IVF because I did not feel comfortable about the risk of multiples that come with injectables + IUI.  Of course, I had also had 9 IUIs at this point so I wasn't convinced they were going to work under any circumstances.  And we have full insurance coverage for fertility treatments, so cost was not a factor.

    As Joy said, weighing the risks of multiples against the desire to get pregnant is a very personal decision.  I would say you should make sure you do research into the costs/risks of having multiples before you make a decision either way.

  • Hi PrincessLeash, I'm sorry about your BFN. I'm also on femara second cycle, going in for 6th IUI this week. Injectables are an option for us in the future if this one doesn't work. However, my RE said she will not proceed with IUI after injectables if I over respond and make more than 4 eggs. I'm still undecided and will cross that bridge when I come to it.

  • We did 3 cycles without injectables and two as a hybrid cycle (utilizes injectable but not the whole cycle-- much cheaper). Those were the two times we got pregnant. The first conception resulted in twins, but we lost the first one very early on and the second one at 10 weeks. We are currently at the end of the 13th week with a singleton and things look really good. Maybe you could ask your doc about a hybrid cycle? It may lower the cost. And be sure to check with your insurance, if you have it, because in our case fertility procedures weren't covered but there was some coverage for medications. 

    Before we went ahead, though, we talked about what we'd do in the case of multiples. We decided we could do twins, but more would be a problem. Our doctor gave us the statistical chances of 3+ and we felt OK with it.

  • Thanks for the responses ladies. I'm not sure what injectables they would recommend. The nurse just said $500 give or take. DW really wants to avoid twins, so I don't think we will do it this cycle and our RE re-evaluate protocol after 3 cycles. So far I'm only getting 1 good follie by ovulation on 2.5 mg of Femera and I'd really like at least 2 to up the odds a bit. They said that upping the Femera won't necessarily give me more follies and they don't need to up it because I'm ovulating just fine. Boo. I guess I just have to trust the professionals and hope that nature will take it's course. But with only 3 more vials of our dear donor (he's totally sold out and retired. We bought the remaining 5 hoping that would get us two kids) it's hard to relax!

  • If you are very attached to this donor and/or having bio siblings, that really changes the risk/reward equation. In that case I'd move on to IVF with only three vials. But like others said, it's a very personal decision.

    In general I really like the three tries and move on approach, but it's fairly conservative. 

    Same sex couple TTC with donor sperm.  I am 35 and carrying.  Endometriosis and DOR.
    AMH 0.5, AFC 5-8, FSH 7ish

    IVF #1 - antagonist.  Empty follicle syndrome.  1 retrieved, 0 fertilized.
    IVF #2 - antagonist.  Ovulated early.  3 retrieved, 2 fertilized, 0 blasts

    Partner IVF #1b - February 2014 - H's eggs, my Ute - ER February 4 (10 retrieved, 3 fertilized), Transfer Feb 7 of one Grade 1 and one Grade 2 day 3 embryos.  1 - Day 3, Grade 1 frosty saved.   BFP - 6dp3dt via FRER, Beta #1 - 110, Beta #2 175, Beta #3 - 348, Beta #4 - 2222!, Beta #5 - 4255.  Ultrasound (6w1d) - 2 heartbearts!  

    We lost our beautiful Twin baby girls on June 18, 2014.  Tavin Sara and Casey Elizabeth were born at 21 weeks gestation and were absolutely beautiful, precious, amazing babies.  We miss our daughters every day and love them with all our hearts.

  • Totally. When our doc mentioned injectables for next cycle since it looks like Manada is only producing one big egg on Femera I had a mini freak out. Twins maybe. But quads, no thank you. : That said, the reason we would move to injectables is M's diminished ovarian reserve, which makes the possibility of quads on a low dose, very small.

    We're queer. I'm 33, have severe stage 4 endo, and had both fallopian tubes removed. My love ("Manada" on the boards, 32) was diagnosed with diminished ovarian reserve. We did Partner IVF (my eggs, her uterus). We lost our twins Tavin and Casey at 21 weeks gestation.

    Our IUIs
    with @Manada: IUI# 1-7 (December 2012- September 2013) all BFN. Tried natural, femara, clomid, puregon/follistim, clomid and menopur combo, both the ovidrel and HCG triggers.

    Our IVFs:
    IVF #1 my eggs November/December 2013: Cancelled IVF due to poor response

    IVF #2 my eggs/Manada's uterus January/February 2014
    BCPs and lupron overlap Stimmed: 1/22-2/2: Bravelle and Menopur (dosage ranged from B300 and M150 to B375 and M150 to B300 and M225)
    2/4 retrieved 10 eggs. Endo was much worse than expected. Only 3 eggs fertilized; February 7 transferred two day 3 embryos, froze one. All great condition.
    BFP eve of 6dp3dt; Beta 1 (11dp3dt): 110; Beta 2 (13dp3dt): 175; Beta 3 (15dp3dt): 348; Beta 4 (19dp3dt): 2222; Beta 5 (21dp3dt): 4255
    1st ultrasound (3/6  6w 1d): TWINS!!!! Twin A measuring 6w1d with a heartbeat of 118bpm. Twin B measuring 6w0d with a heartbeat of 113bpm.

  • Just got off with my insurance company and it looks like we have 80 coverage for injectables. Now it's really tempting to use them. We're especially DW are terrified to have twins. My grandma had twins and dear donor has twin brothers. Yikes! What to do!!??

  • We went to injectables after Clomid failed me. Injectables failed me too. We are now homestudy approved for domestic infant adoption. Oh and the upshot was the injectable wrecked such havoc with me that I gained at least 35 pounds and was miserably sick for the entire cycle and a few months after.
