applying for disability

I've never been on this forum before so I apologize if someone had posted about this already. I was wondering how to go about applying for NYS disability for when I have my baby? I'm 99% sure I'm not getting approved my family leave from work but I was told I could still get disability for 6 weeks. I also heard if my work "makes" me quit I could possibly get unemployment. I've looked it up and unemployment seems to be a gray area.

    I live in NY but worked in NJ, so I can't help too much as my disability came from NJS. I got all of my paperwork from my human resources department. There were three parts: 1 for me to complete, one for HR and one for my doctor. I ended up resigning so I don't have any advice about unemployment. Good luck!
