Use the screams to your advantage. When he screams, shoot the medicine at the back of his mouth (back-side helps to not have coughing). He'll swallow before he screams again. It also helps to pin him down and have him mostly laying down for this, but standing also works. I prefer to only give small amounts at a time (in case it gets spit out), say 1 ml, but DH does the whole 5ml of Advil at once this way. It's not the nicest way to give medicine, but it does work. Have you tried aiming the syringe at the inside of his cheek instead of the back of his mouth. Makes spitting out the medicine that much harder.
DD1 once had antibiotics she really hated, and the only way we got her to take it was by mixing it up with chocolate syrup. A mom's gotta do what a mom's gotta do, right?
I would suggest mixing small amounts of it with a bite of something he likes like applesauce. I think they get freaked out by the syringe. Or maybe even just giving it to him with the spoon would work.
Not sure what type of dropper you're using, but we got one of these (click here for link) syringes to replace our original dropper. It really helps because you can shoot the medicine out quicker. We wait till DD opens up to wail, and then shoot it back towards the inside of her cheek. And like Mama Ha said, if you need to mix it with a sweet treat, do what you gotta do!
