Multiples

I know NOTHING about Inductions....

....And I kind of want to know what to expect from real-life perspectives. I realize everyone is different but I'm so curious as to how other MoMs inductions went.

I'm scheduled to go in the night of 12/10 for cervical ripening (as of today I wasn't dilated at all) and then be induced the AM of 12/11. I wonder what the heck to expect after that. The told me Pitocin...that's it. Of course, the doc and midwives don't deal with multiples as intimately as you all have so I'd like to pick your brains. TIA!

  • seriously?  They couldn't explain the induction any better than that? You don't have an MFM?  When I was induced with my singleton they started in me a pitocin drip. checked on me every so often.  Are your babies head down?

  • The ripening agents they'll put on the cervix are used to help you dilate/efface. The pitocin helps start and/or move contractions along. Pitocin-based contractions are very strong compared to "natural" contractions - I do highly recommend an epidural if they are going to give you pitocin, but they often make you wait for the epidural until you've made at least several centimeters of progress. Epidural is generally a good idea with twins anyway, from what I hear, in the unexpected event that you end up having to have a Cesarean for the second (or even both) babies. I haven't had the twins yet, but I was partially induced with my first singleton (my water broke, but I had no contractions and was only dilated to 3 with "halted labor"). The pitocin drip was started, but they wouldn't apply any agents to my cervix since my water had already broken. I was expected to walk about the hospital with the drip going to try to speed labor, but all that did was piss me the F off... a few hours later I convinced them to give me the epi despite my lack of progress in dilation, and fell asleep promptly after receiving it. Next thing I knew, I was woken up a few hours later and told we were ready to push. Epi + sleep = happy productive labor, for me. :)

  • I went in the night before for antibiotics because I was GBS+ (dilated to a 2 or 3 at that point). I asked the dr if we could do Cervidil (cervical ripening agent) at the same time and he agreed. The next morning (dilated to a 4 after the night of Cervidil) they started the Pitocin drip around 9:45 a.m., broke baby A's water around 1:45 p.m. (was only at a 5 after 4 hrs of Pitocin), and was fully dilated and ready to push by 5:45. ETA: I got the epidural around 3:45. The babies were born at 8:43 and 8:50 p.m. Pushing A out took forever and B flipped to breech again so he came out by breech extraction.

    Yes, seriously lol. I could have asked a lot more questions but there are so many different docs and midwives that I can never get a straight story and they always seem to change my plans depending on the doc.

    I have an MFM but not at my hospital - I go to the MFM facility only to have scans and won't see them during labor.

    Both babies are head down and who knows what can happen between now and then but I have a DSS that DH and I need to plan around...trying to figure out when DH should be at the hospital if I still require cervidil. Sounds like it takes a while if cervidil is needed. As it turns out, since posting, my inlaws are coming into town this weekend for a week and will be able to watch DSS while DH is with me.

     I appreciate the personalized stories b/c what the docs say are "supoosed" to happen and what happens never seems to be the same Wink

  • I went in for my induction at 7pm. After about an hour of check-in stuff and monitoring, they inserted the first dose of Misoprostil, since I was not dilated at all (well, a "fingertip," which might as well be nothing.) It caused cramping, kind of like bad period cramps. After four hours I was between 1 and 2cm.  They inserted a second dose, which also led to more cramping.  Four hours after that I was at 4cm and having contractions.  The doctor was hesitant to order pitocin because I was in active labor, but while the contractions were not very strong, they were very frequent.  She was afraid that giving me the Pit would cause me to contract almost constantly.  But weighing the options she went ahead and ordered it.  At this point I got my epidural so it would be in effect when the Pit started.

    My awesome L&D nurse, also a twin mom, put off the Pit as long as she could.  She left to attend the delivery of her other patient, and told me she would start the Pit when she got back.  While she was gone my water broke.  The was enough to get the contractions really going, so I never got the Pitocin.  Somewhere in the 4:00pm hour my doctor and nurse came rushing in, checked me, and pronounced me 10cm and ready to push.  They threw scrubs at my DH and Mom and wheeled me to the OR (my hospital requires that all twin deliveries be in the OR).

    What I didn't know at the time (and am grateful that they didn't tell me) was that my Baby B was in distress.  I only pushed about five times to deliver Baby A.  He was whisked off, and the doctor looked me in the eye and said, "You need to have this baby RIGHT NOW."  That was my first real clue that something was wrong.  She pulled him down into position (he tried to come out hands first) and I delivered him in 1 push.  The boys were born at 5:15pm and 5:20pm.

    Baby B was fine once he was out - they think his cord was being compressed during contractions.  Both babies were fine, no NICU.
