Depressed but need info on SLOWEST flow nipple...

I saw an LC to discuss problems with my 2 week old LO who is inconsolably upset after each feeding. It looks like I will need to supplement with either formula or pumped breast milk afterall. I am absolutely devastated as I?d tried as hard as I could to make sure I used no artificial nipples with my LO as much as possible. But he clearly needs more to grow and thrive than I?ve been able to provide. It breaks my heart to see him so upset and hungry after each feeding...

So I want to know where I can find the SLOWEST flowing artificial nipple as possible? I want to do as much as I can to avoid having my LO have nipple confusion. I?m hoping that I can build up my supply and bring him back to my breast eventually.

Any advice? I am feeling like such a failure and am hoping that if I can at least find a good artificial slow flow nipple I wo'nt have to worry about this all going downhill...

  • My hospital used to carry PUR nipples (they don't anymore-but they may be available online) and they were fantastic! Slow flow and baby had to "latch on" to get the milk out. We used it with my daughter who had an inefficient suck. I also had to supplement because she just wasn't able to empty my breast effectively. I would nurse for as long as she wanted, then gave 1/2 to 1 ounce of pumped BM. Then I would pump after.

    It only took a short time (maybe 2 weeks) for the combo of her increased sucking and my increased supply from the pumping to fix our problem and go back to EBF again. It's disappointing, but you do what you need to make sure baby is happy and healthy! Good luck- I hope you get back to EBF soon!

  • A supplemental nursing system would be a great way to supplement at the breast so your body still gets the stimulation it needs to get the signal to make more milk.

    Did the LC do a weighed feeding?

    When my LO gets a bottle, we use Dr. Brown's with preemie flow nipples.

  • Dont worry--i had to do the same at day 4 with pumped BM and LO has been fine and no nipple confusion ever. Still BFing at 2 mos now! hang in there, it gets better :) Adiri makes a great breastlike bottle...

  • Have you read on kellymom about giving a bottle? Nipple confusion has been debunked so don't worry about that. Relax and it will be ok!

  • Have u tried a nipple shield? My daughter is 4months old and drinks only pumped breast milk as she simply wouldn't nurse. I tried everything but she prefers the bottle and at the end of the day the breast milk is what she needs so I don't worry too much about how she gets it as long as she getting the mommy milk. For the nipple soreness use nipple ointment all natural so u don't have to wipe it off after every pump and don't pump on the highest/fastest setting. U can work ur way up to high during ur pump to help prevent soreness but while u have the blisters u should avoid the high setting as much as possible. So sorry to hear u are having problems right now but stick with it ur milk is the best thing u can feed ur baby and it will get easier...hope some of this helps.

    Is the LC you saw an IBCLC lactation consultant with experience outside of the hospital setting with newborns?
    Was your baby evaluated for tongue tie, including side and posterior? high palate?
    what was your diaper count with baby just nursing?
    what is pre and post nursing session weigh in amount? How much milk did baby transfer at the breast?
    was latch evaluated well?
    there is something going on aside from low supply for generic reasonsmy guts tells me that if the latch issue isn't fixed, it doesn't matter what you do to your supply, baby will continue to struggle to get enough milk.
    of course I could have you mixed up with someone else.

    I agree. I think you need to get another opinion on the tongue tie, if I remember correctly you had it checked but was told it was fine. Keep pushing for an answer because something is not right!

  • Hi everyone. Thanks for your responses.

    Yes, I am the mom with the sore, red nipples that come out lipstick shaped when LO nurses although this seems to be an issue more on the right side. 

    Yes, he was screaming frantically yesterday after feedings although today he seems much more content after feedings. I'm not sure why. I DID notice though that I experienced the letdown sensation for the first time last night and have had it about 3 times since then right before feedings. I'm hoping this might be a good sign and that LO is getting more?? I have no idea. 

    Yes, I have had peds as well as 3 different LC's (all IBCLC certified) evaluate him for tongue tie and they have all been unanimous in noting that he does not have this problem. 

    Yesterday  when he was frantic was the first time his wet diaper count was off - only 4 wet diapers the entire day.

    We had taken him to the peds and were told that he had lost an ounce (after initially gaining 6 ounces since being discharged from the hospital) but the pediatrician reassured me that it was nothing to be concerned about. He suggested we have him reweighed on Monday. 

    I'll have to see how this goes. I appreciate suggestions for bottles, SNS etc.  I'm relieved to know I'm not alone with these issues. I have really tried to push through all the issues and continue to exclusively nurse so the thought of having to supplement really upsets me.

    BTW I have not tried a nipple shield b/c I'm afraid of it affecting my supply. That and I'm not sure if there are shields big enough for my large nipples!

    ok, so how much milk did baby get at the breast when nursing?

    if you do not know this, then it's really really hard to proceed and protect your breastfeeding relationship.

    Prob the fussiness was baby working hard to up your supply during a growth spurt and getting really worked up. 

    losing weight is a problem, but 1 oz can be the difference between a pre poop or post poop weigh in, an ounce is not very much.

     

    This is what I would do.  put baby to the breast every 2 hours around the clock for the next 24 hours.  use breast compressions and massage to get baby to stay on side #1 the longest possible.  then burp for several minutes and offer side #2 as dessert.  Use asymetrical latch or laid back nursing to get a deeper latch, past the soft palate curve so perhaps nip won't be so sore and uneven shaped.  With low diaper count, I would offer 1 oz of pumped breastmilk by syringe, finger feeder, supplemental nursing system, cup, spoon, eyedropper, or slow flow bottle nipple(choose premie size).  then pump for 5-15 minutes after the milk stops flowing to stimulate a higher milk supply.  use what you pump for the next supplement.  If you get where you are pumping more than an ounce after nursing I will estimate that your supply is NOT the issue, and that latch and milk transfer is the issue.

    After the 24 hours, re-evaluate.  you should see increase in number of diapers, and increase a bit in amount pumped.  prob then go to 2 hours during the day between feeds for a day or two, and let yourself have a 3-4 hour rest at night. then go go 2-3 during day, and 3-4 at night.  I would stay at supplementing about 1 oz per feedig and that rate of frequency of feeding until baby is gaining weight well OR until you can see an LC that will do a pre and post nursing weigh in on a very accurate scale so you can see baby is getting approx 25 oz a day.

    if the amount you get pumping increases, like I said supply is prob not your issue.  you would then begin to back off on supplements and watch diapers.  If you have 6-8 wet and or poopy diapers a day then baby is getting enough to eat.

    So to clarify you recommend supplementing 1 oz at each feeding? BTW I have tried supplementing at night with a syringe and all he does is scream when I try to give it to him that way...
