Modified Bedrest discussion with OB - WWYD? (Long)

At my appointment earlier this week, my doctor decided to do a quick u/s as my belly was really low and I told him I was having some minor pressure in my pelvic area with increased BH.   He said both boys are head down and have dropped.  He decided to do an internal and said I am not quite 1cm dilated but my cervix is still long and is not effaced basically just no longer closed tight. 

Due to this he told me it was getting to a point where we may need to take me out of the field for work as I am on my feet for 10+ hours when in the field (work solely from home) and talk about modified bed rest just as a precaution.  Then he got called to an emergency delivery so I was unable to finish my conversation with him.  The nurse practitioner came into the room to finish my appointment and I asked her if my OB said anything about needing to start modified bed rest (up no more than 4 to 6 hours a day I think is what he said) and she said all he told her was we would take another look at things at my next appointment (Dec. 10th). I took this as we might be nearing that point but we weren't there yet so continue as normal until told otherwise.

I was out in the field all day yesterday visiting customers.  This means I was in and out of my car and on my feet at these meetings. I drove about 250 miles and was gone from home about 11 hours.  By 4pm I felt like I couldn't stand up straight and there was a bowling ball between my legs.  By the time I got home BH where happening pretty frequently.  At this point I couldn't sit comfortably or move without wanting to cry. I had been drinking water all day like well more than a gallon. I decided to take a bath to see if I could get it to calm down and after soaking for about 30 minutes the BH calmed way down (I still was having them but back to the regularity of days I am home) and the pressure was almost back to the normal level I had been feeling.  I think I just way over did it.

I completely understand listening to my body I just don't know if these are things to be concerned about.  I plan on calling my OB if it happens again tomorrow while out in the field.  I know the doctor says go to L&D if anything feels off but as this is my first pregnancy I am not exactly sure what is off and what is way over did it aka calms down after bath or rest. Stick out tongue

My question is was anyone else by 28 weeks starting to dilate/soften and still working long hours on your feet (3+ days per week) with your doctor ok with this?   As a precaution should I go ahead and ask to be taken out of the field?

I am not sure if dilation at this point in a multiple pregnancy is normal or cause for concern.  Any experiences or advice you can share would be greatly appreciated.  Thanks to anyone who made it this far reading sorry it was so long!

  • At 28 weeks, there's no way I would have felt up to doing that kind of work! Even if I hadn't started to go into PTL. I say slow it down. It sounds like your body is trying to tell you the same thing. You don't want to get to a point where resting and baths don't help slow down your contractions. I'm a full advocate for preventative measures. :)

  • Please call your doctor. My PTL started exactly the same way at 28w1d and I was hospitalized on bedrest. My PTL felt just like I had over done it and was having a lot of BHs.

  • I would put in a call to your doctor and get the rest of the conversation completed ASAP.  Get it clarified what he means.  My take away from that was he wanted you to slow down ASAP, not after a recheck. The recheck is to make sure it's enough. You want those babies in safe and growing for another several weeks.

    TTC since May 2006. After 3 failed Clomid cycles, 2 failed Injectibles/IUIs, 2 failed IVFs and 1 failed FET, we moved on to adoption!

    this is what I thought he meant based on your post.  I wouldn't return to a full day in the field until you have finished this conversation with your ob. at 28 weeks I was exhausted working an office job with my feet up most of the time...no way I could have kept up with the day you are describing. gL!

  • Oh I would definitely get in contact with your doctor and go in to get checked. Then I would urge them to put you on restricted bedrest asap. I really think that 1cm dialated at 28 weeks is dangerous when you are talking about twins. It's time to take it easy - really. Being on your feet so much is very dangerous in a multiples pregnancy and is one of the leading causes of premature labor. I don't mean to scare you but please call your doctor - go in - and tell them you need to slow it down. And then slow it down! Take it easy! It's so important for you and your babies.

    I just left a message for him with the nurse.  She did tell me that if I had that kind of pressure again to go to L&D immediately if nothing else to make sure it's not causing my cervix to change.  She said "You have carried them this far so now it's really time for better safe and sent home than PTL that goes too far to stop.  It's easier to stop it the faster we catch it so next time just go".   That made me feel quite dumb about not going last night!

    Note to self: Call or go if I feel anything past uncomfortable.   I honestly thought I was just making a big deal of the normal 28 week discomfort of a twin pregnancy.

    Thank you again ladies!  Now to bake these two for at least 9 more weeks!

    This.  You don't want to always wonder "what if".   I'm not saying you need to spend the next 9 weeks laying horizontal but I wouldn't push yourself at all now.

    I agree completely, you've got to slow way down... you need those babies to stay put as long as possible, it's your chance to give them their best possible start on life!

    I went to ld twice and other than the bill ugh don't regret it at all. The second time I went I was having timetable contractions at 36 weeks which scared the poo out of me. It's always better to go and not need them than to need them and not go.

     I went to L&D 4 times before we delivered. Don't feel weird about it; that's why it's there!
