Hi ladies,
Thanks again to all who responded to my intro & novel last week. My OB's office did call me back last Wed, but my Dr. was on call and had a nurse call me. We went back and forth on the phone until pretty late in the evening as she was just not listening to what I had to say (or willing to be very helpful) and my Dr. was eventually called out on an emergency.
Initially she told me he wouldn't be back in the office until Wednesday of this week, but after a bit of harshness on my end, conceded that he would be working a half day Friday and she would try to get him the message.
Another nurse called me Friday morning and said that my Dr. wanted me to go ahead and start with the progesterone suppositories. I explained again that I have been having two lh surges a couple days apart (with corresponding temp dips/rises) the past few months and that not only was that concerning to me in general, but it made it difficult to know when to start the progesterone. Like I said before, I'm nervous that if I think the first surge is a fake and wait for the second, I'll miss needed days of the meds, but don't want to start on the first surge if there's a second and have the progesterone interfere with ovulation. More than that, I feel like the progesterone is going to address a symptom of the problem, not the problem itself.
She said she would go talk to the Dr. again and call me back, which she did, and said that he wanted me to start it 4dpo and that I needed to use the first surge and ignore the second as "it's not possible to ovulate twice in one cycle and most people stop testing after the first positive anyway." I was so mad. Obviously I know that you can't actually o twice in a cycle and understand the hormone surge, etc. I tried to explain that it seems like my body gears up to o, doesn't, and then tries again a few days later (weak ovulation?) and she said that's not possible. She also said that with my cycle length (28-29 days) I needed to go off of the first surge at cd13-14 because you're "supposed to start your period about 10 days after you ovulate so it wouldn't be possible for the surge on cd17-19 to be ovulation." I wanted to throat punch her. A short lp is the reason they wanted me to try progesterone in the first place, a 10 day lp isn't "normal" and 13 plus 10 doesn't equal 28!
I was so frustrated, but after five days of phone calls and getting nowhere, I just felt so defeated and hung up with her. Now I'm not sure what to do. I've considered switching practices, but my OB group is the only practice for the area serving the hospital that I prefer and the closest RE is two hours away. That being said, I'm not even sure if seeing an RE is justified at this point.
I'm definitely not going to start on cd14 though - while one Dr. told me to start taking the progesterone from O on, the other told me to take it from 4dpo on so hopefully if I just start taking it after that second surge (assuming it happens again this month), I'll have my bases covered.

Thanks, ladies. Kristin, yes I do chart with FF and have been o'ing on cd 17-19'ish since my first loss in June. The last two months, I've had an LH surge prior to that, which apparently my doctor's nurse has never heard of.
I'm just nervous (knowing how things can change so quickly) that I'll ignore that first surge waiting for a second and then it won't happen. Not much I can do about it I suppose! Fingers crossed that nothing else changes this month and I'll start the progesterone as soon as I confirm O.
Oh, and when I explained this all to the first nurse (the first surge not resulting in O) she said "Well, if that's the case then you're basically doomed." Nice.
What a douche.

If I were you, I'd be looking for a different doctor... Or maybe find some good medical literature that points out how completely uninformed this nurse is?
Exactly! If I had good options I'd be out in a flash, but I don't want to make a hasty move that will come back to bite me. In the meantime, I'll begin "project make the nurses look stupid."
Pretty sure they cover false/multiple LH surges in TCOYF (I gave my copy away and right now I'm kicking myself for it)... Send them a copy, maybe?

It does sound like you like your doctor, but the nurses are not good. I would also schedule an appt, bringing your chart and I would complain about the nurses. TTC and TCCAL is so stressful and emotional and they need nurses that are understanding to that. GL

I am so sorry you are going through this with your doctor. I don't really have any advice I just wanted to say I was sorry that you are dealing with all of this on top of your loss. Big Hugs!
