Good morning! I've gone through the last couple days of posts and I don't see this so please let me know if its already out there and ill look further.
I'm working on my birth plan and am looking for some examples if anyone would like to share theirs.
My hospital is already very natural birth friendly so I don't have to put too much on there but would still like to see some formatting and verbiage examples.

I) LABOR
- We would like to avoid continuous electronic fetal monitoring
-We would like to avoid vaginal exams until the urge to push
- Please refrain from offering for pain medication of any kind
-We plan to move freely and labor in whatever position is comfortable
- No rupture of membranes or Pitocin to move labor along
-We would like to avoid an IV but will allow a hep lock to be put in
II) Delivery/Birth- No medical interventions unless absolutely necessary (to be discussed b/t husband and I privately before any decision is made).
-we prefer to tear naturally and would like to avoid an episiotomy
-We will push in any position I feel comfortable, squatting, kneeling, side position
- Want immediate skin to skin contact with the baby
-Please delay cord clamping for 3-5 mins after birth
-Husband would like to cut the cord once it has stopped pulsing
-Clean and take care of baby on chest, no immediate removal for weight or eye ointment
-Baby allowed to breast feed immediately following birth
-Once baby has breast feed (@ 1-2 hr) then eye ointment and shots can be administered
-Pitocin and any other drug can be administered to stop hemorrhaging or help shrink uterus
-Any drug can be administered to numb vaginal area and stick tears

I might be too late to help you but this is mine. I have a full length with the "please" and the niceties, but then I made it into bullet points for the general, if everything goes well parts. Where I focused was on the details of the "if something goes wrong / hospital transport" stuff that I can't communicate before hand to my providers. Note: this may still change, I'm only 1/2 way to the big day, but my midwives haven't read through all of it either.
Summary of Home Birth Birthing Plan for (Me)
? Non-midwife attendees include Husband, Sister, Close Friend
? I want to keep moving; encourage non-reclining positions to be more comfortable during labor
? Minimal internal examinations, but I am ok with them as needed.
? Do not offer to break my waters
? After 4cm dilation I may want to labor in a birthing pool
? I may want to deliver my baby is a birthing pool; upright, kneeling, supported squatting or all-fours. I do not want to be on my back or semi-reclining unless that position seems right for me.
? I do not want to deliver the placenta in a birthing pool
? Do not offer pain relieving drugs unless I ask for them
? Pain management options I?d prefer: movement, massage, relaxation, a TENS machine, or a birth pool.
? I will only push when I physically cannot do otherwise though.
? Help me to avoid perineal damage if possible
? Do not announce the baby's sex
? Do not clamp the cord until it stops pulsating, do not pull on cord or administer drugs without permission.
? If husband wants to cut cord, guide him.
? We are keeping the placenta.
? I will breastfeed my baby as soon as possible after the birth.
? Keep the baby unclothed and close to my skin immediately after birth
? No injection of Vitamin K, use the oral Vitamin K.

In case of transfer to hospital:
If transfer by ambulance becomes necessary, please do not strap me in on my back - I would prefer to be on my side, to make contractions easier to deal with.
Please do not offer Pethidine/Demerol/Stadol as I am concerned about its possible effect on my state of mind in labor, and on my baby's health. I would prefer to have an epidural.
I do not agree to student doctors being present during my treatment.
I am very keen to avoid a caesarean section if it is at all possible, as I do not want to enter future labors with a scarred uterus. I particularly would like to avoid a caesarean just for slow progress. If labor is simply taking a long time but my baby is not in immediate danger, please would you encourage me to keep going and perhaps to try changes of position or movements which might help. Please do not offer a caesarean unless my baby or I, or both of us, is in danger.
If a caesarean section becomes necessary, I would prefer to remain awake with epidural or spinal block anesthesia. I would like (husband) and a midwife to stay with me at all times, and would like to breastfeed the baby immediately after birth.
If there are limits to the number of non-hospital people allowed in my room during labor and delivery priority goes in this order: Husband, Lead Midwife, Second Midwife/Student Midwife, Sister
In the event of a c-section, if there are limits to the number of non-hospital people allowed in my room during delivery priority goes in this order: Husband, Lead Midwife. I want the Lead Midwife present during closure to observe multi-layer closure of the incision.
If there are problems after the birth:
If I suffer a severe post-partum hemorrhage, please do not perform a hysterectomy unless it is the only available course of action. I still hope to have more children.
Please do not admit my baby to Special Care simply for observation, but only if there is a specific reason for concern.
I would prefer my baby stay with me for the first hour, even if I am still in surgery from a c-section.
In the event of a c-section, I do not want my uterus sewn up in one single layer but multiple layers, I intend to attempt vaginal delivery with future pregnancies.
Baby Feeding
My baby will be breast fed only. Please do not give my baby supplements of glucose water or formula milk without my written permission. I would expressly like to avoid having any formula supplements unless it is unavoidable, and certainly not in the first two days in any event.
Under no circumstances is my baby to be bottle-fed.
If I have trouble breastfeeding, I would greatly appreciate the help of a midwife who is fully supportive of breastfeeding, and would also like to be given contact details for local breastfeeding counselors.
