A friend made ours with iron-on transfers--she created and printed her own (you can buy iron-on transfer paper at craft stores), but I'm sure you could buy transfers pre-made, too.
I got a little crafty with this. I went to JoAnn fabric and bought some transfer paper. I printed from Martha Stewart website the #'s on to transfer paper and am in the process of ironing them on right now. I'm hoping they look good!

I think most people just use stickers. You can buy them on Etsy or from various other places online. The ones I have are Sticky Bellies brand.
We were given a set of monthly stickers from a friend who found them on Etsy.
I picked up a set of plain white Gerber-brand onesies from Target that I adhere the sticker to for the monthly picture.
