I know most tummy time mats come with a pillow, and my doctor suggested using one, but dd doesn't seem to like it. Anyone know any reasons I should use one versus letting her lay directly on her tummy?
I often use the Boppy for tummy time - my LO then has a view of what's going on and can look at pictures/books. He is happier on the Boppy than on the playmat without a boost. Flat on the mat, he's just face-planted and he gets frustrated easily.
With DS, I used the mat with the little pillow for tummy time. He would get really frustrated if I left him flat for too long. We would only do it in small increments.
Now with DD, she loves being on her tummy and it seemed like only a couple of days before she started holding her head up during tummy time. She would go to sleep on her tummy with head turned, if I left her. Of course I won't, but she loves it.
Every kid is different. I would just follow your LOs lead.

My son rarely used a pillow and he hit all his gross motor skills early (sat unassisted at 4.5 months, crawled at 6 months, etc.). There really is no need for one. I've heard of people suggesting them if your LO doesn't tolerate tummy time, that it sometimes helps them enjoy it more. You can use any pillow for that, but the Boppy/nursing pillow works great. You could even roll up a receiving blanket or towel I'm sure. Something that props them up and their arms can drape over. But yeah...flat on the floor (I prefer to lay a blanket down first) is perfectly fine.
