Filing for De Facto Parent Status

Hi all, it's been a long time since I've posted over here, we are traveling a rough road trying to adopt our girls and I need some advice.

We've been advised by Advokids to file for de facto and I have no idea what kind of info to include on the JV296. How detailed should I be in section 6?
