VBAC

wound on head of OP baby? VBAC success

I'm looking for guidance and to see if anyone was in my situation. I'm not pregnant, but just planning for the future...

With my first, who was sunny side up (OP?) and had a head circumference in the 93%, after pushing for 5 hours I needed a c-section, as he was stuck on my pelvis and ended up with a wound on his head (posteriorlateral, from hitting my pelvic bone).  What I am wondering, is if it was my body shape and I should assume that a RCS will be necessary, or if it was his positioning and head size that ultimately lead me to the CS and if I can try for a VBAC. 

Thanks for any guidance.  My OB says that I can try for a VBAC and she thinks I have a 50/50 chance given the duration of time that I pushed for.

  • I had a similar situation with my son.  My OB has given me a 55% chance of a successful VBAC based on the size of my first child and my age.  It is my understanding that each baby/birth is different and you can't really go by the baby "not fitting" the first time.  I have heard of women having a successful VBAC with a baby that was 2 lbs. bigger than the CS baby that would not fit.  I think there's just no way to tell unless you try.  Frustrating, I know.

    Hopefully others on here will have helpful experience but I just wanted to let you  know I am in the same boat.

    the pelvis spreads and moves and if everything is aligned properly it could fix the issue you had with your son.

    also spinningbabies to make sure baby is OA, an OA baby is going to fit much better than an OP baby.

  • I think your chances are pretty good, probably better than 50/50 even. Positioning is a really, really common reason for a CS. 

    My first was OP, and I pushed for 4+ hours with him. He had a huge bruise (hematoma?) on the top of his head for weeks after he was born, from how he was positioned when I was pushing.

    My VBAC baby was 2+ lbs and inches bigger than his brother (10.5 lbs and over 23"), and I did push two hours with him, but I that had a lot more to do with his size than my pelvis or anything. 

    I have two older sisters who both had similar first births - pushed for hours, and baby wasn't line up well, and ended up with a c/s. My oldest sister opted for RCS with her other kids, but my other sister also went on to have a successful VBAC with her second.

    Do you feel like your doctor is really supportive of VBAC? My MW never doubted that I wouldn't have a VBAC with my second, even with how my first labor went.

  • thanks for all the support.  I feel like my OB is mildly supportive of VBAC, I really like her, and she will leave the option up to me, but might decide to split my appointments with her and the midwife of the practive (whom I know is supportive of VBAC). 

     

    I am a PT, and am pretty sure my pelvic alignment was okay during labor/delivery, although that was a problem previously, and since I had an epidural, who knows!

  • What kind of wound was it?  Broken skin or just bruising?  Bruising is pretty common for babies who are posterior or in an unfavorable position, and I don't think it usually hurts them.

    My VBAC baby had a huge lump on the side of his head, which was bruised, from the way he was in there.  Pushing was long (almost 4 hours), but he came out with no tearing.  The bump went away in a day or two, the bruising was there for a few more days, but also went away.  It didn't seem to bother him.

  • his wound was broken skin, about the size of a quarter.  by the time they cleaned him up it scabbed over and we never needed to do anything to it.  not sure if others experienced an actual wound or not?
