I don't see why you would need another name, to me John is the FN and David is the MN, even if he'll always be JD

Yes this. There is no need to add another name in the mix. I have a friend named John Michael. We called him that growing up. In high school he just started going by John.
I was the OP in the to hyphenate or not hyphenate 2 middle names post. I think in this case I would use just John David Lastname. If calling him John David as a whole, I don't think it matters if the David is considered a middle or a second first. Think Lisa Marie Presley. You always hear the Lisa Marie as if its 2 firsts even though Marie is the middle.
Now if you did choose a separate middle name, I think certain sounds or names could work. Just like with me using 2 middles names they all have to sound good together which can be harder when using multiple. Although my first boy name will never be said with either 2 middles obviously.
I like the two first names my sister and myself have two ans we have a middle name. People have always commented on how unique it was. Julie Ann Marie and Carlie Marie Belle

As a girl, she was always called "Sara Jane." Even when I was little, I can still remember my grandparents referring to her with an even mix of "Sara Jane" and "Sara" although most people just use her first name now.
John David Lastname, called "John David" seems perfectly acceptable to me. No reason to stick another name in there and confuse everything.
I have been considering a two part first name for a girl and I think I would skip the MN entirely. That could be because I can't think of another name I like with it. In sum it is already 4 syllables.
I think John David LN is enough, even though I really like Arthur too.

The one person I know IRL with 2 MNs says it's a pain. I'd imagine 2 FNs would be even worse.
