I moved DD1 when she was 2. She wasn't necessarily showing signs of "readiness" like climbing out of her crib or anything. But, DD2 was on her way, and I was having a c-section so I knew I couldn't lift her out of her crib. We converted her crib into a toddler bed, and the transition went really well. It really wasn't a transition at all b/c she never cried or even seemed to notice.
At 18 months, DD decided she hated her crib and bedtime became a battle, so we moved her to a full sized bed. It took about 2 weeks of watching her on the monitor and waiting outside her bedroom door to get her used to staying in her bed(she would try to get out of bed and we would just return her to bed).
We switched from crib to twin bed at exactly 2 1/2. She loved her crib and never climbed out, but with a new baby on the way we needed to use it for him. About a month before we talked it up big time....we seriously talked about her getting a big girl bed every day to really pump her up. I showed her pictures online, etc. One weekend when she stayed with my mom we totally transformed her baby room into her big girl room. She came home and was so excited and surprised. It was a very smooth transition. I was scared that it would be horrible, but thankfully she proved me way wrong
We converted DS1's crib to s toddler bed around his 2nd birthday. He was in our room and we just had to remain attentive and ask him to go back to his bed - still do even though he's in his own room now. We'll be moving him into a full sized bed in a few monthss when DS2 outgrows the cradle and needs the crib.
DD1 transitioned herself at 23 months. We had planned to do it at 24-25 months so we could give DD2 the crib when she was 6 months, but set the bed up in her new big girl room early so she could get used to the idea. One day she asked to nap in it, slept great, and we never looked back! Super smooth transition, she actually doesn't get out of bed without permission. We're not sure why she thinks she needs permission to get up, but we aren't shattering that idea anytime soon

We transitioned to a twin bed at 17 months. DS was climbing out of his crib several times a night at that point and I was too nervous to leave him in a crib. The transition went smoothly and it was the best thing we could have done for his sleeping. He wasn't sleeping through the night in the crib and started as soon as he went into the twin bed.
He hadn't been climbing out (although he had climbed in a couple times). Our main reason was due to him starting daycare & we thought having him in a bed at home would help with nap time on cots at daycare.
We haven't had trouble keeping him in his room. When he wakes up, he typically stays in his bed & calls for us.
One DS's 2nd birthday. He got a whole new room and bed and we made it a big deal. (We needed the crib for #2 and wanted a little time where the nursery was empty)
We put the mattress on the floor with some toddler rails on it so it still feels a little closed in. Then we got very lucky and told DS he had to wait until mommy or daddy came to get out of bed. And he did. So now I hear "Oh Mommy" at the top of his lungs in the morning but he stays in bed.
My friend had trouble keeping her daughter in her room and put a doorknob cover on the inside and just made sure the room was a safe as possible.
Re: At what age did you transition from crib to 'big kid' bed?
DD1 moved to a twin bed at 22 months when she started climbing out of her crib.
DD2 is 2y2m and still in her crib.
She'll be in there for another 6 months, and then we'll have to start transitioning her.
