If your DH has seen a urologist

Our appt is next week and I'm wondering what to expect.  I'm going to bring his SA results.  Anything we should ask specifically?  What kind of testing do they usually do? TIA!

BFP #1: 2/15/12-Ectopic/Mtx at 6 wks

BFP #2: 10/12/12-m/c at 5 wks. 
BFP #3: 8/27/13
Me: MTHFR, Low AMH (1.1), High NK cells and Antiphospholipid Antibodies.
DH:MFI-Low morph (1%), DNA fragmentation: Excellent!
5 rounds of clomid, 3 IUIs, and multiple non-medicated cycles- BFFN                                                      
IVF with ICSI in August 2013 brought us our babies.  ER-9R, 7M, 4F w/ICSI  ET of 2-Grade 2 blasts. 
+ HPT at 6dp5dt. #1 (8dp5dt)- 105    #2 (13dp5dt)- 510  #3 (15dp5dt)- 960  #4 (17dp5dt)- 1889
Dx shortened cervix, PTL, and preeclampsia during pregnancy.  Lots of medications and 13 weeks of bedrest, babies were born healthy at 34w4d!

  • My DH and I did things a little backwards (he went to urologist before I went to RE) but your DH should be prepared for a full exam.  Mine was pretty shocked and struggled with it for a week or so.  It all depends on what they find - the UA showed blood in my DH's urine, so he had to have a CAT scan and cystoscopy done as well.  Hopefully that won't be the case with yours!

    Cycle #1: Clomid 100mg + Ovidrel = No Response, Canceled
    Cycle #2: Clomid 150mg + Ovidrel = No Response, Canceled
    Cycle #3: Clomid 150mg + Follistim + Ovidrel = No response, Canceled

    Cycle #4: Femara 7.5mg + Gonal-f + Ovidrel + IUI = BFP!!!!

  • My DH had a physical exam & got orders for a repeat SA as well as labs to check his hormone levels.

    Me:29 DH:29  TTC since 1/11 Dx: unexplained IF/early DOR/immune issues 
    Feb'12- July'12-testing(all clear minus slight arcuate ute), 3 IUI with clomid all BFN
    8/30IVF#1 Antagonist protocol- ER 9/11-8R, 7M, 5F. 
    ET 9/14 2 embies transferred. 1 10cell Grade 4, 1 8cell Grade 4. No frosties. BFN
    IVF#2 Antagonist protocol plus baby aspirin- ER 12/5-16R, 12M, 8F!
    ET 12/10 5dt! 1 fully expanded blast & 1 early blast. No frosties. BFN
    3/13 hysteroscopy & polypectomy, Consulted w Dr. Kwak-Kim.  
    DX: High NK cells, cytokines, DHEAs& PAI1;  hypothyroid, +APA, restricted bloodflow
    7/13 IVF#3 Long lupron protocol with PIO, Crinone, Prometrium, and vivelle
    (plus synthroid, metformin, baby asa, metanx, PNV, Vit E, D, calcium, fish oil, CoQ10, IVIg infusions and lovenox per Dr. Kwak-kim)
    ER 7/19 14R, 11M, 9F(4 natural fert, 5 with ICSI)
    ET 5dt 7/24 2 fully expanded blasts.  SURPRISE 3 FROSTIES!!!
    Beta #1 8/2 335!!!! Beta #2 829!!!  1st u/s 8/14 showed TWINS!!!!!

  • Physical exam, discussion if there are any surgical options (varicocele), changes in diet, lifestyle, vitamins, medications.  You may also want to ask about a blood test for testosterone levels.

    I hope they can give you answers.

  • Good thinking to bring his SA results.  Our uroligist is in the same hospital system as our RE, so he already had the results there.  We went over medical history a bit, then MH got both a front and rear exam.  TMI alert, he wasn't expecting to get his prostate checked, so warn your hubby ahead of time that might happen.  MH was diagnosed with a varicocele upon examination and a nurse came in and set up the appt.  He didn't want a repeat SA done until the varicocele was taken care of.  No bloodwork for MH.

  • My DH has not actually been to a urologist yet, but if his SA results are the same on the second SA he will be making an appointment.  However, I worked with the urologist in the OR one day and asked him what to expect.  He said that he will do a normal history and physical, a testicular exam (looking for varicocele, congenital absence of vas deferens, ect...), and will often order blood work for a hormone evaluation.  He said that he also likes to sit down and educate patients on the possible causes and what to expect, ect...  I hope this helps.

    Off BC since Febuary 2006 3 Clomid Cycles in 2008- symptomatic, US just showed a tilted uterus, otherwise normal, HSG normal.  DH has an ejaculatory duct obstruction and a congenital absence of the vas deferens.  This is common with cystic fibrosis carriers, but he doesn't have cystic fibrosis. IVF #1 with TESE/ICSI (Antagonist protocol) Aug 2014=BFN (poor embryo quality)   IVF #2 ICSI long lupron protocol November 2014

  • Yup, bring the SA results and questions you have for him. He will check out hubby physically to look for varicoceles. If the SA came back abnormal then he will ask questions like if he ever had surgery or an injury down there before. He may want to perform a blood test to check hormone levels too.

     At our appt, we found hubby had a medium varicocele so we discussed surgery. He already had two SAs and his hormone levels tested, so we talked about those results and asked what his opinion was regarding treatment (he thinks IUIs will work). We also got a script for clomid and more b/w and more SAs in the future to see if its working for him.

    Overall it'll be you three chatting in the exam room and at some point your hubby will drop his drawers for an examination, then put them back on and keep chatting :) One of the least scary meetings with the doctors I think! Good luck to you guys :) 


  • My DH had never had a prostate exam before so the urologist did that for him as well as the regular check.  I think the prostate exam was more disturbing for my DH than anything.  ha!
