I don't remember it being an application... but honestly my HR lady took care of everything. I do remember that I had to use all my PTO before it would kick in. She gave me a sheet to give to the doc and it's determined by the type of delivery you have. I had a c/s so I should've gotten 8wks, so that's what they paid me. Well then my fabulous doc returned me to "work" after 6 so I had to actually pay them back b/c they already sent the check to me based on the type of delivery I had. I didn't actually go back to work so I didn't have to worry about FMLA or anything.
I have NO Idea where to start. She told me to contact an old coworker (who doesn't even work with me anymore) to find the forms. She has no idea what the process is.

I'm sorry I can't say with 110 certainty, but I think I brought a paper to the hospital to be filled out or signed by the doc when he was born this is how you are put on disablility, it has the birth info on it. Then I know I brought the other paper to my followup 6wks and it was my release back to work.
I used to work for a doc and fill out those papers all the time for patients but it's been almost 2 1/2 yrs. I'm 99 sure that's how it goes. Usually with disability there are forms to be filled out every 4wks or so until you're released back to work. As far as where to get the forms and where to send them I don't know b/c your employer is supposed to handle all of that from what I remember both with my experience and those of patients.
I'm sorry they're not being more helpful, is there an admin you can contact about that? Does she make you do your own payroll too, lol!? Just kidding... But for cereal she sucks.
For my company for the temp disability I had 3 forms, one for me to fill out and send in, one for me to fill out and have HR send in and one for the doc to fill out after birth and send in. I believe the last form is basically the same for everyone, the doc form, but the other two not sure.

I'm not completely sure because I haven't done it myself, but a couple years ago I had to employees that I had to relay this information to. If I remember correctly, I was told that you should have all the paperwork filled out ahead of time but that you can't actually turn anything in until after the baby is born. Our hr person suggested having it all set so that DH or whoever could apply immediately after the birth and you don't have to deal with it while you're busy recovering.
