TTC After a Loss

MTHFR

After my miscarriage, my doctor did testing because there is a family history of blood clots. So she did the first wave of tests and the only thing that was back was that I am compound heterozygous for MTHFR. She suggested for me to start taking extra folate (which I did). She then said that heterozygous is typically not thought to be related with MC and that it is greatly debated topic. She said that she was probably being over cautious but she was going to just another round of test, homecystiene, protien C and some other protiens and some othe clotting panels cant quite remember them all. She said if anything came back abnormal she would send me to a specialist. Well everything came back normal. Which i was happy to hear, but i am kind of nervous about the mthfr. I am happy that I am taking the extra folic acid, I feel like I should take something else. I have done some research myself I have found the same things that my doctor told me. Anyone else heterozygous other labs normal and just take folic acid?

  • You only need to really be concerned about MTHFR is you are homozygous (have 2 mutated copies), not hetero - the majority of the population is actually hetero. Many dr. will prescribe the extra folic acid &/or a B complex as a precaution. Some may also recommend baby aspirin if they think it's necessary (only do this if your dr has recommended it), but if your homecyteine levels are in normal range, it really shouldn't be much of a concern.

  • Yep--I have hetero C677T MTHFR.  I've had 2 OBs and both say they do not believe it is related to my losses.  But as a precaution, I took 4mg FA, B6, B12, BA, and my prenatal.  GL!

     

    ETA:  It's actually 40% of the population have hetero MTHFR, so not quite the majority.

  • Even though heterozygous MTHFR is common, you still have a limited ability to metabolize synthetic folic acid.  Your MTHFR enzyme (which breaks down folic acid) works at 30% capacity with homozygous MTHFR and 70% at heterozygous.  This puts you at risk for folate deficiency related pregnancy complications.  Active L-methylfolate is already broken down and bypasses the MTHFR mutation.  You might want to ask your doctor about it.  L-methylfolate is 7 times more bioavailable than synthetic folic acid.

  • I just saw this while doing a MTHFR search. I was diagnosed with compound heterozygous last week. Like you all my other panels came back negative. She has me on baby aspirin and 15mg Deplin. I'm a litte nervous about it too, but happy we may have answer to what's causing my m/cs. Good luck to you!

    It sounds like your doctor is being very proactive given your history of RPL.  With the amount of L-methylfolate you are taking you can be sure that MTHFR-related folate deficiency will not be a problem.  Best of luck!
